1. Structure of the Respiratory System |

The human respiratory system consists of the nasal cavity, trachea, bronchi, bronchioles, and alveoli. The trachea branches into two bronchi, which further divide into smaller bronchioles ending in clusters of air sacs called alveoli. The ribs, intercostal muscles, and diaphragm support the mechanics of breathing.

The air enters through the nasal cavity where it is filtered, warmed, and moistened. It then passes down the trachea, which is supported by C-shaped cartilage rings to keep it open. The trachea divides into two bronchi (singular bronchus) that enter the lungs. Inside the lungs, bronchi branch into smaller bronchioles. The bronchioles end in tiny air sacs called alveoli, which are surrounded by a dense network of capillaries.

The trachea and bronchi are lined with ciliated epithelial cells and goblet cells that secrete mucus. The mucus traps dust and pathogens, and the cilia beat to move the mucus upwards toward the throat to be swallowed or expelled. This self-cleaning mechanism helps protect the delicate alveoli from infection.

2. Differences Between Inhaled and Exhaled Air |

Inhaled air contains approximately 21% oxygen, 0.04% carbon dioxide, and a small amount of water vapour. Exhaled air has reduced oxygen (around 16%) and increased carbon dioxide (about 4%), with saturated water vapour. The nitrogen content remains largely unchanged at about 78%.

These differences arise because oxygen diffuses from the alveoli into the blood, while carbon dioxide diffuses from the blood into the alveoli to be exhaled. The air is also warmed and gains moisture as it travels through the respiratory passages. The body does not use nitrogen, so its concentration stays constant.

3. Gas Exchange at the Alveoli |

Gas exchange occurs in the alveoli by diffusion. Oxygen diffuses from the alveolar air space into the blood capillaries because the oxygen concentration in the alveoli is higher than in the deoxygenated blood. Simultaneously, carbon dioxide diffuses from the blood (where its concentration is higher) into the alveoli. The gases move down their concentration gradients without requiring energy.

As oxygen dissolves in the moist lining and crosses the thin alveolar and capillary walls, it enters red blood cells and binds to haemoglobin. The resulting oxyhaemoglobin is then transported to body cells. Carbon dioxide produced during respiration travels in plasma and is released into the alveoli to be exhaled.

4. Adaptations of Alveoli for Gas Exchange |

Alveoli are adapted for efficient gas exchange in several ways:

1. Large surface area – millions of alveoli provide a combined surface area of approximately 70 m² in an adult, greatly increasing the rate of diffusion.

2. Thin walls – each alveolus wall is only one cell thick (made of squamous epithelium), minimizing the diffusion distance for gases.

3. Moist surface – the inner alveolar surface is covered with a thin film of moisture, allowing gases to dissolve before diffusing through the membrane.

4. Rich blood supply – a dense network of capillaries surrounds each alveolus, maintaining a steep concentration gradient by constantly carrying away oxygenated blood and bringing deoxygenated blood.

5. Elastic fibres – the alveolar walls contain elastic fibres that enable the alveoli to stretch during inhalation and recoil during exhalation, helping to force air out.

5. Mechanism of Inhalation (Breathing In) | 吸气机制

Inhalation is an active process. The external intercostal muscles contract, pulling the ribcage upwards and outwards. At the same time, the diaphragm muscle contracts and flattens. These movements increase the volume of the thoracic cavity. As volume increases, the pressure inside the lungs decreases below atmospheric pressure. Air is drawn into the lungs to equalize the pressure.

During deep or forced inhalation, additional muscles such as the sternocleidomastoid and pectoral muscles may also help raise the ribs further, increasing the inspiratory volume. The abdominal muscles relax to allow the diaphragm to move downwards easily.

6. Mechanism of Exhalation (Breathing Out) | 呼气机制

Normal exhalation at rest is mostly passive. The external intercostal muscles and diaphragm relax. The ribcage moves down and in, and the diaphragm returns to its dome shape. This decreases thoracic volume, increasing the pressure inside the lungs above atmospheric pressure. Air is forced out of the lungs.

During exercise or forced exhalation, the internal intercostal muscles contract to pull the ribs down more forcibly, and abdominal muscles contract to push the diaphragm up further. This produces a more rapid and complete expulsion of air.

7. Effects of Exercise on Breathing |

During exercise, muscle cells respire faster, consuming more oxygen and producing more carbon dioxide. The increased CO₂ level in the blood is detected by chemoreceptors in the medulla oblongata. These receptors send impulses to increase the breathing rate and depth, leading to higher minute ventilation. This helps meet the oxygen demand and remove excess CO₂.

Breathing rate can rise from about 12 breaths per minute at rest to over 30 during vigorous exercise. Tidal volume also increases, and the extra air intake boosts the oxygen supply to muscles while removing metabolic waste. After exercise, breathing remains elevated for a period to repay the oxygen debt and clear lactic acid.

8. Lung Volumes and Spirometry |

A spirometer trace shows different lung volumes. Tidal volume (TV) is the volume of air moved in and out during a normal breath (about 0.5 dm³). Inspiratory reserve volume (IRV) is the extra air that can be inhaled after a normal inhalation. Expiratory reserve volume (ERV) is the extra air that can be exhaled after a normal exhalation.

(RV) remains in the lungs to prevent collapse and keeps alveoli slightly inflated; it cannot be measured by a simple spirometer.

During exercise, the trace shows increased tidal volume and breathing rate, resulting in a higher minute ventilation. Exam questions often ask you to calculate TV or breathing rate from a given trace.

9. Effects of Smoking on the Respiratory System |

Cigarette smoke contains harmful chemicals such as tar, nicotine, and carbon monoxide. Tar destroys the cilia lining the airways, so mucus and trapped pathogens cannot be swept away. This leads to a persistent smoker’s cough and chronic bronchitis, where the airways become inflamed and narrowed.

Tar also damages the alveolar walls, causing them to break down and merge into larger, irregular sacs. This reduces the total surface area for gas exchange – a condition called emphysema. Sufferers become breathless even after minimal exertion because their lungs cannot oxygenate blood efficiently.
