Vague essays sink otherwise solid files, because a reader working through dozens of applications notices immediately when the only stated reason is a wish for change. Late transcripts and unexplained gaps in enrolment do the rest, and for international applicants a technically sufficient but non-compliant bank document can end the file without any academic judgement at all.
