GCSE Edexcel
Health, disease and the development of medicines,考纲把两件事绑在一起: 疾病为什么会发生,以及人类用什么办法把它压下去。
Topic 5 of the Edexcel GCSE Biology specification sits in Paper 1 (1BI0/1F and 1BI0/1H), a 1 hour 45 minute paper worth 100 marks and 50% of the qualification. This article compares the two families of disease side by side, then connects the immune response, vaccines, antibiotics and drug testing to the way the mark scheme is written.
一、健康不只是没有症状: WHO 的四个维度 / Health is more than the absence of symptoms
答题时把 three dimensions 里的身体、心理、社会三维都点到,再补一句 not merely the absence of disease 就完整了。
Specification point 5.1 asks for the WHO definition of 1948 word for word: health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity. Marks disappear when a candidate reads health as a purely medical condition, because an item about a student with no symptoms but severe anxiety then looks unanswerable; naming the mental and social dimensions is what earns them.
Two different causal chains
Communicable diseases are caused by pathogens and pass between individuals, or from animals and vectors to humans, which covers tuberculosis, malaria and HIV. Non-communicable diseases are not transmitted at all; they accumulate from genes, lifestyle and environment over years, as with cardiovascular disease, type 2 diabetes and most cancers. Comparison items reward the candidate who separates "can it spread" from "is a pathogen involved", never the one who ranks the two groups by how serious they sound.
Four pathogen groups and six named infections
Pathogens fall into four groups: bacteria, viruses, fungi and protists, and point 5.5 names six infections you should be able to place. Cholera is a bacterium causing diarrhoea, tuberculosis a bacterium damaging lung tissue, Chalara ash dieback a fungus producing leaf loss and bark lesions, malaria a protist damaging blood and liver, HIV a virus that destroys white blood cells and leads to AIDS, and stomach ulcers come from Helicobacter bacteria. The usual slip is filing malaria under bacteria, so learn each one as pathogen, target organ, symptom.
Transmission routes and the controls that match them
A pathogen often has several routes at once: cholera travels in contaminated water and food, tuberculosis in droplets, Chalara spores on the wind and in infected saplings, malaria through Anopheles bites, and HIV through sexual contact, blood and from mother to child. Every control measure has to match a named route, which is why handwashing scores nothing for malaria while draining standing water, spraying and bed nets do.
Why one disease raises susceptibility to another
Point 5.3 asks you to explain why one disease makes others more likely, and examiners dress it in a new context most years. HIV destroys lymphocytes, so infections that a working immune system keeps in check, such as tuberculosis and pneumonia, take hold. Chalara damages bark and lets other fungi and insects in, while malnourished patients make fewer antibodies and white cells and heal slowly. Write the mechanism, since "the immune system is weak" on its own is worth a single mark.
Risk factors behind non-communicable disease
Smoking delivers tar, carbon monoxide and nicotine, damaging alveoli and the lining of blood vessels, and it is a leading risk factor for lung cancer and coronary heart disease. Long-term alcohol intake damages liver cells and leads to cirrhosis, while diets high in fat, salt and sugar, together with obesity, push up blood pressure and LDL cholesterol. The specification calls these risk factors rather than causes, because lifelong smokers sometimes escape lung cancer.
七、BMI 的数字能说明什么、不能说明什么 / What BMI shows and what it hides
BMI divides mass in kilograms by height in metres squared. The NHS puts adults below 18.5 as underweight, 18.5 to 24.9 as healthy, 25 to 29.9 as overweight and 30 or above as obese, with lower thresholds of 23 to 27.4 for people of South Asian or Chinese background. Its weakness is that it cannot separate muscle from fat, so a teenage weightlifter can be labelled overweight, which is why waist-to-height ratio is used alongside it.
Treating cardiovascular disease: stents, statins and lifestyle
When fatty plaques narrow a coronary artery the heart muscle is starved of oxygen and angina appears, and a complete blockage is a heart attack. A stent holds the narrowed section open and restores flow within minutes, but it only deals with damage already done, whereas statins slow the liver's cholesterol synthesis and lower LDL over months. That difference in timescale is the answer to the usual comparison question about why both are used.
The immune response: phagocytes, antibodies and memory cells
Once a pathogen gets past the first line of defence, phagocytes engulf bacteria without targeting a particular species. Lymphocytes then recognise antigens on the surface of the pathogen, and only the clones that fit that antigen divide and release matching antibodies; the primary response takes days to peak, which is exactly when the patient feels ill. Memory cells remain, so a second exposure produces antibodies within hours.
Vaccines and herd immunity
A vaccine introduces killed or weakened pathogens, or just their antigens, so the body runs a primary response and keeps memory cells without suffering the disease. WHO's 2023 measles factsheet states that about 99% of people who receive two doses are protected for life, and that 95% two-dose coverage in a population stops large outbreaks. That is how unvaccinated children stay safe while coverage holds, since the surrounding immune barrier breaks the chain of transmission.
Antibiotics act on bacteria only
Antibiotics attack bacterial cell walls, protein synthesis or DNA replication, and viruses offer no such target, so using them on a cold brings side effects and resistance pressure with no benefit. Resistance is not a bacterium learning a trick: a few individuals already carry a resistance allele, the drug kills the susceptible majority, and the survivors multiply and pass the allele on. WHO's antimicrobial resistance fact sheet puts bacterial resistance at about 1.27 million deaths directly in 2019, and mutation always comes before selection.
Drug development: placebo, blinding and trial phases
A candidate drug is first tested on cells and tissues for toxicity, then on animals for dose and side effects, and only then in human trials. Phase one usually uses healthy volunteers to find a safe dose, phase two looks for benefit in patients, and phase three compares a larger group against controls. The placebo group receives an inactive treatment so that feeling cared for is not mistaken for a drug effect, and random allocation spreads age and severity across both arms.
How to structure the comparison answer
Mark schemes for these items work in pairs, so each point should appear as "communicable diseases do this, whereas non-communicable diseases do that" rather than as two unrelated lists. Extended answers run through definition, example, mechanism and conclusion, and a six-mark item usually wants three linked mechanisms plus one named example. Piling up terminology without the causal steps stalls a script at half marks.
