AQA GCSE
Infection and response is unit 4.3 of the AQA GCSE Biology specification (8461), first taught from 2016, and it covers pathogens, the body's defences, vaccination, antibiotics and drug development. The wording is deliberately restrained, with almost no statistics to memorise, only the routes of transmission, the layers of defence and the reasons a medicine works. Questions still tend to open with a scenario, such as a measles case in a class, and ask what should happen next.
Pathogens are the microorganisms that cause infectious disease, and the 8461 specification divides them into viruses, bacteria, protists and fungi that infect both animals and plants. Bacteria multiply quickly inside the body and release toxins that damage tissues, which is why a bacterial infection often brings fever, abdominal pain or diarrhoea. Viruses have no cell structure of their own and can only replicate inside a host cell, destroying it in the process. The specification limits transmission to direct contact, water and air, so answers need to name the route in context rather than repeat a definition, and malaria is the classic trap because the pathogen is a protist while the mosquito is only the vector.
Practical control measures look ordinary in daily life: handwashing, cooking food thoroughly and treating drinking water block the food and water route, while isolating patients, wearing masks and ventilating rooms target droplets in the air. Vector control works differently, because draining standing water, sleeping under a net and using repellent stop the mosquito from biting in the first place, and section 4.3.1.1 asks for these concrete actions rather than a vague mention of hygiene. Vaccination reduces the number of susceptible hosts, so a pathogen that cannot find its next host breaks its chain of transmission. A common mistake is to present antibiotics as prevention, which fails twice over because they do not touch viruses and they are no substitute for immunisation.
Measles is a viral infection marked by fever and a red skin rash, and it spreads when droplets from coughs and sneezes are inhaled; complications can be fatal, which is why young children are routinely vaccinated. HIV starts as a flu-like illness and then attacks the immune cells, and without antiretroviral drugs the damage eventually reaches the stage called AIDS, where the body can no longer deal with other infections or cancers. The specification names sexual contact and the exchange of body fluids such as blood through shared needles as the routes. Tobacco mosaic virus infects plants including tomatoes, producing a mosaic pattern of discolouration that lowers photosynthesis and stunts growth.
Salmonella food poisoning comes from contaminated food, or food prepared in unhygienic conditions, and the fever, cramps, vomiting and diarrhoea are caused by the bacteria together with the toxins they secrete. In the UK poultry are vaccinated against salmonella, which is a source-level control rather than a treatment for patients. Gonorrhoea is a bacterial sexually transmitted disease with a thick yellow or green discharge and pain on urinating; penicillin once treated it easily, but resistant strains have appeared, so antibiotics now go together with barrier contraception such as condoms. Many students assume diarrhoea always points to a virus, yet bacterial food poisoning is just as common, and naming the pathogen type earns the mark more reliably than describing the meal.
Rose black spot is a fungal disease that produces purple or black spots on leaves, which often turn yellow and drop early, cutting the photosynthetic area and weakening the plant. It spreads through the environment in water or wind, and treatment combines fungicides with removing and destroying affected leaves instead of leaving them near the pot. Malaria is caused by a protist whose life cycle includes the mosquito, so the insect is the vector rather than the pathogen, and control means stopping the insects from breeding while using nets to avoid bites. Many students write that killing mosquitoes removes the pathogen, when the insect only carries the protist from one person to another.
The body has several defences that do not target a particular pathogen. Intact skin is a physical barrier, and sebum and sweat keep its surface slightly acidic, while mucus and hairs in the nose trap inhaled particles. Cilia in the trachea and bronchi sweep mucus and trapped microorganisms upwards so they can be coughed out, and stomach acid kills most of the microbes swallowed with food. Because these barriers work against any invader rather than one specific type, they are called non-specific defences, and the specification only asks for the skin, nose, trachea and bronchi, and stomach. A common exam error is to treat cilia and mucus as one structure, when cilia move the mucus while mucus traps the particles.
Once a pathogen gets past the barriers, white blood cells take over in three distinct ways. In phagocytosis a white blood cell engulfs the pathogen and digests it with enzymes, a step that works against any invader rather than a specific one; lymphocytes produce antibodies that bind to antigens on the pathogen surface, marking the cells or clumping them so phagocytes can clear them. Antitoxins neutralise the toxins released by bacteria, which relieves symptoms even before the bacteria are destroyed. Many students describe antibodies as killing pathogens directly, but they mark and neutralise rather than kill.
A vaccine introduces small quantities of dead or inactive pathogen so that white blood cells produce the matching antibodies, and after the pathogen is cleared the body keeps memory cells that recognise the same antigen. On a second exposure those cells respond faster and in greater numbers, so the infection is stopped before symptoms appear; the specification deliberately leaves out vaccination schedules and the side effects of individual vaccines. At population level the effect depends on proportion, because when enough people are immunised the pathogen has fewer chances to pass between hosts and unvaccinated people gain indirect protection, which is herd immunity. Many students think a vaccine kills the pathogen itself, when its real job is to train the immune response.
Antibiotics such as penicillin cure bacterial disease by killing the infective bacteria inside the body, and a given bacterium needs a specific antibiotic. They cannot kill viruses, because viruses replicate inside host cells and a drug that damages them tends to damage body tissues as well, which is why viral infections are harder to treat. Painkillers relieve symptoms such as fever or pain without killing the pathogen, so a patient can feel better while still carrying the infection. Resistance emerges through selection, since a few bacteria already carry gene variants that make them less sensitive, and when the drug kills the rest those survivors multiply into a resistant strain.
Traditional medicines were extracted from plants and microorganisms, with the heart drug digitalis coming from foxgloves, aspirin from willow and penicillin from the Penicillium mould that Alexander Fleming identified. Most new drugs are now synthesised by chemists in the pharmaceutical industry, although the starting point may still be a chemical taken from a plant. Before any human use, a candidate is tested for toxicity, efficacy and dose; preclinical testing uses cells, tissues and live animals in the laboratory, and clinical trials then begin with healthy volunteers at very low doses before the optimum dose is established. In a double blind trial neither the patients nor the doctors know who receives the drug and who receives the placebo, which removes the effect of expectation.
这部分内容在 8461 spec 里标注为 biology only 与 higher tier only,Combined Science 的学生不考。
Monoclonal antibodies come from a single clone of cells and bind to one site on one protein antigen, which lets them target a specific chemical or a specific cell in the body. They are made by stimulating mouse lymphocytes to produce a particular antibody and fusing them with a tumour cell to form a hybridoma, which both divides and secretes that antibody, so cloning single hybridoma cells yields large amounts of identical antibody. Uses range from pregnancy tests and blood assays to research imaging and cancer treatment, where the antibody carries a radioactive substance, a toxic drug or a growth-blocking chemical straight to the cancer cells. The specification marks this material as biology only and higher tier only, and monoclonal antibodies cause more side effects than early hopes suggested.
Diseased plants show recognisable signals such as stunted growth, spots on leaves, areas of decay, growths, malformed stems or leaves, discolouration and the presence of pests. Identification can rely on a gardening manual or website, a laboratory that identifies the pathogen, or a testing kit containing monoclonal antibodies; the specification limits plant pathogens to tobacco mosaic virus, rose black spot and aphids. Ion deficiencies are easily mistaken for disease, because a shortage of nitrate ions removes the raw material for protein synthesis and stunts growth, while a shortage of magnesium ions prevents chlorophyll formation and causes chlorosis. Plant defences are physical, chemical or mechanical, covering cellulose cell walls, a waxy cuticle and bark as barriers, antibacterial chemicals and poisons against herbivores, and thorns, hairs, drooping leaves or mimicry as mechanical adaptations.
