📚 How the UK Prepares Young People with Special Educational Needs for Adult Life | 英国如何帮特殊教育需求青少年准备成年生活

In England, around 1.7 million pupils - roughly 18 per cent of the school population - are identified as having special educational needs (SEN), and about 640,000 of them hold an Education, Health and Care Plan (EHCP). For these young people, leaving school is not a single event but a planned process that begins in Year 9 and can legally continue until the age of 25. The UK has built one of the most detailed transition systems in the world, combining statute, statutory guidance, local authority duties, health commissioning and employment programmes. This article explains how that system works, where it succeeds, and where it still fails.

1. The Legal Framework |

Transition planning in the UK is not voluntary good practice; it is a legal duty. Four statutes do most of the work: the Children and Families Act 2014 (Part 3) for England, the Equality Act 2010, the Care Act 2014, and the Mental Capacity Act 2005. Together they create rights to assessment, to reasonable adjustments, to adult care and to supported decision-making.

The Children and Families Act 2014 replaced SEN statements with EHCPs and extended the system from 0-19 to 0-25. This single change is the cornerstone of adult-life preparation: a plan can legally follow a young person out of school and into college, training or supported employment.

The Equality Act 2010 imposes a duty on schools, colleges, employers and service providers to make reasonable adjustments and not to discriminate against disabled people - including by treating them less favourably because of something arising from their disability.

The Care Act 2014 requires local authorities to carry out a transition assessment for young people who are likely to need adult care and support, and to do so before they turn 18 so that no one falls through the gap. The Mental Capacity Act 2005 applies from age 16 and establishes the presumption of capacity, the duty to help someone decide, and best-interests decision-making when capacity is absent.

Devolved nations differ: Wales operates the Additional Learning Needs (ALN) system under the 2018 Act; Scotland uses the Additional Support for Learning framework; Northern Ireland uses the SEN Framework under the 2005 SENDO. Comparisons between them are a common exam question.

2. The SEND Code of Practice and the 0-25 System | SEND

The SEND Code of Practice: 0 to 25 Years (2015) is statutory guidance issued jointly by the Department for Education and the Department of Health. It tells schools, colleges, local authorities and health bodies exactly what they must do. Its central principles are participation, early identification, integration of education, health and care, and preparation for adulthood from the earliest years.

The Code creates a two-tier model. "SEN support" is the internal, school-based level for pupils who need extra help but not a legal plan. An EHCP is the higher, statutory level for pupils with more complex needs. Roughly 13 per cent of pupils receive SEN support and about 7 per cent hold an EHCP.

The Code also introduces the Local Offer - a published, accessible summary of every service available in a local authority area for children and young people with SEND aged 0-25. In theory it lets families plan without guesswork. In practice, quality varies widely, and Ofsted/CQC area inspections have repeatedly criticised weak Local Offers.

Level 层级Who it covers 适用对象Legal status 法律地位
Needs met by differentiated teaching and in-school interventions

An EHCP is a single legal document, usually 10-25 pages long, with a fixed structure. It must contain the young person's views, interests and aspirations; their special educational needs; the outcomes sought; the special educational provision required; any health or social care provision; and the name and type of the school or college.

EHCP = SEN + Health Needs + Social Care Needs → Specified Provision → Agreed Outcomes

The "outcomes" section is where adulthood preparation lives. Weak plans write vague outcomes such as "to improve social skills". Strong plans write measurable, forward-looking outcomes such as "to travel independently to college by bus three days a week by July 2026, using a travel-training programme". Inspectors and tribunals both look hard at this section.

EHCPs must be reviewed at least annually and must be maintained until the young person's 25th birthday if they remain in education or training. However, a plan ends when the young person leaves education or training, enters higher education (where Disabled Students' Allowance takes over), or takes up paid employment of more than 20 hours a week that is not a supported internship or apprenticeship.

Local authorities issue, maintain and fund the plan. Parents and young people aged over 16 can request an assessment, request a particular school or college, and appeal to the First-tier Tribunal (SEND) if the local authority refuses to assess, refuses to issue a plan, or names a placement they disagree with.

4. The Year 9 Review and the Transition Plan |

The Code of Practice is explicit: the annual review held in Year 9 must include a focus on preparing for adulthood. From that point on, every review must consider the four adulthood outcomes, record the young person's views, and identify the support needed for each post-16 stage.

In practice, transition planning means naming a lead professional, inviting adult services to the review, agreeing a timeline, and building skills long before they are needed. Independent travel training, for example, usually takes 6-18 months and should start in Year 10, not in the summer before college.

Key practical milestones typically look like this: Year 9 - transition discussion begins; Year 10 - college and provider visits, travel training, work experience; Year 11 - placement named in the plan, application submitted; Year 12-13 - review of progress, adult care transition assessment; age 17-18 - Care Act transition assessment completed, adult health services identified; age 18-25 - plan maintained in further education or supported employment.

Critics note that in many areas Year 9 reviews remain paperwork exercises, with no adult social care representative present. The Care Act 2014 was designed to fix this, but local authority budget pressure has limited how consistently it happens.

5. The Four Preparing for Adulthood Outcomes |

The Preparing for Adulthood (PfA) framework, developed with the National Development Team for Inclusion, is the organising idea behind English transition work. It defines four outcomes that every plan should build towards.

These four areas interact. A young person cannot hold down a job without travel skills, and cannot travel confidently without managing anxiety - a health outcome. Good transition planning therefore works across all four simultaneously rather than treating them as separate checklists.

6. Post-16 and Post-19 Options | 16

England offers a wide - and often confusing - menu of destinations after compulsory schooling. Under the Raising the Participation Age rules, young people must continue in education or training until 18, which covers most of the transition period.

EHCP can be maintained EHCP可继续维持
General FE college 普通继续教育学院Mixed levels, Entry to Level 3 各级别混合Largest destination for EHCP holders EHCP持有者最主要去处
Young people with EHCPs wanting paid work 想就业的EHCP持有者Study programme + unpaid work placement + job coach 学习项目+工作场所实习+就业辅导员
Paid; Access to Work may apply Access to Work

Supported internships, introduced in 2012-13, are widely regarded as the single most effective school-to-work route for young people with EHCPs. They last at least six months, usually a year, and around half of participants move into paid employment with their host employer or elsewhere.

7. Supported Internships and Employment Support |

Employment outcomes for disabled adults in the UK remain poor. Only around 5 per cent of adults with a learning disability known to social services are in paid work, compared with roughly 52 per cent of disabled people overall and about 75 per cent of the non-disabled population.

Several mechanisms exist to close that gap. A job coach provides on-site training and then fades support as competence grows - the "place, train, maintain" model of supported employment. Access to Work funds practical support such as specialist equipment, travel costs, a support worker or a communicator, and there is no fixed upper limit for most claimants.

Other schemes include the Disability Confident employer accreditation, the Work and Health Programme, and specialist providers such as the British Association for Supported Employment (BASE) network. DFN Project SEARCH, which places interns entirely within a single host employer such as a hospital, reports paid employment outcomes of around 60-70 per cent in some cohorts.

Paid employment rate: learning disability ≈ 5% < all disabled ≈ 52% < non-disabled ≈ 75%

The gap illustrates the central criticism of UK transition work: the system produces good plans more often than it produces good jobs.

8. Independent Living, Housing and Personal Budgets |

Independent living does not necessarily mean living alone. It means having choice and control over daily life: where you live, who you live with, how you spend your money, and who supports you. Transition planning therefore covers cooking, budgeting, using public transport, staying safe online, and understanding consent and relationships.

Housing options after 18 include living with family, supported living (a tenancy plus a care package), shared lives arrangements (living with an approved carer family), extra care housing, and residential care for those with the highest needs. Local housing authorities must give "reasonable preference" to people who need to move on medical or welfare grounds, which can include disabled young people.

Personal budgets and direct payments allow a young person (or their family) to receive money instead of services and to purchase support themselves. In SEND, personal budgets can cover elements of the EHCP provision; in adult social care, direct payments are a well-established option under the Care Act. They increase choice but require administrative capacity that many families find hard to sustain.

9. Health, Wellbeing and Adult Social Care |

Health transition is the weakest link in the chain. While education transition is governed by a clear statutory code, the move from child to adult health services has historically been fragmented, with paediatric services ending at 16 or 18 and adult services not always accepting referrals.

Key protections include the Annual Health Check for people with a learning disability, available from age 14 and recorded on the GP learning disability register; the hospital passport, which records communication needs and reasonable adjustments; and Learning from Lives and Deaths reviews, which examine premature mortality among people with a learning disability and autistic people.

Mental health transition from Child and Adolescent Mental Health Services (CAMHS) to adult services is a particular flashpoint. Many services have an age cut-off at 18, and young people who do not meet adult thresholds can be discharged with no replacement support - the classic "cliff edge".

Under the Care Act 2014, a young person likely to need adult care has a right to a transition assessment, and the local authority must consider whether they have "eligible needs". Assessments can begin before 18 so that services start on the birthday rather than months later.

10. Participation, Rights and Decision-Making |

Participating in your own transition plan is a legal principle, not a courtesy. The Code of Practice requires that young people's views be sought, recorded verbatim where possible, and taken into account. From Year 9 onwards, the young person - not only the parent - is treated as the plan holder.
