Thematic visits into how local areas prepare young people with special educational needs and disabilities for adult life produced a joint report from Ofsted and the Care Quality Commission on 16 December 2024. Its central finding is that the main barriers are systemic and long standing rather than the fault of any single service, and it explains why support thins out as a young person turns 18. What follows is what the visits examined, what the four preparation pathways cover, and what the recommendations mean in practice.
Area SEND thematic visits are joint work by Ofsted and the Care Quality Commission, and each year they pick a single slice of the system to look at in depth; alternative provision in 2023, preparation for adulthood in 2024 and children who are not in school in 2025. The visits are directed by the Secretary of State under section 118(2) of the Education and Inspections Act 2006, and the CQC assists under schedule 4 to the Health and Social Care Act 2008. Inspectors spend up to four days off site and up to four days on site; they form no judgement about an individual area, and the national report names the areas visited without attributing findings to any of them.
The four pathways that frame preparation for adulthood come from the 2014 SEND code of practice: employment, independent living, community inclusion and health. Inspectors looked at children and young people from birth to 25, whether they held an EHC plan or received SEN support in a mainstream setting. Under each pathway they asked whether joint planning actually put a young person's wishes into decisions, rather than whether the paperwork listed every duty.
六个地方分别是 Wolverhampton、Wandsworth、Thurrock、Newcastle upon Tyne、Bath and North East Somerset 和 Wigan。
The report published on 16 December 2024 draws on visits to six local area partnerships: Wolverhampton, Wandsworth, Thurrock, Newcastle upon Tyne, Bath and North East Somerset and Wigan. Inspectors held meetings with education, health and social care partners as well as with young people and families, and they gathered evidence from documents, surveys and visits to providers. More than 2,400 survey responses came in from children, young people, parents, carers and practitioners, and 52% of parent and carer respondents either disagreed or strongly disagreed that children and young people with SEND are given opportunities to fulfil their potential in adulthood.
Careers education, information, advice and guidance was better matched to need in specialist settings, where some schools employ transition workers who are qualified to deliver it. Children with an EHC plan generally received stronger support, and one partnership ran its own service offering bespoke guidance from Year 9 onwards; pupils receiving SEN support in mainstream schools were far less likely to get the same. The report cites National Audit Office figures from October 2024 showing that 69% of pupils with SEN at key stage 4 were in sustained education, employment or training after 16 to 18 study, compared with 85% of those without.
Supported internships recur throughout the report, because they are one of the few established routes into paid work for young people with SEND. Inspectors heard positive accounts from those who took part, and job coaches in colleges, schools and councils were seen as useful; the barrier is eligibility, since an EHC plan is required, and families reported that the plan ends once their child starts work. The report recommends that the Department for Education expand the number and reach of supported internships, and it notes practical obstacles such as travel cards that cannot be used at peak times and wheelchair access problems.
Independent living was assessed through the practical side of adult life: dressing, cooking, managing money and getting around. Inspectors saw early years practice that built independence from the start, multi-agency equipment panels that sped up access to daily living aids, and life skills hubs with dedicated mentors; more than half of the children and young people who responded agreed or strongly agreed that they get support to make their own decisions and to live on their own where appropriate. The recurring constraint was accommodation, as every area reported too little specialist residential and supported living provision, with fewer options still for young people who have a physical disability.
Community inclusion covers friendships, hobbies and being visible locally, and inspectors saw a range of offers: early years hubs for children with additional needs, universal and targeted youth clubs, and short breaks being expanded in one area. Provision is not keeping pace with rising numbers, and much of it rests on short-term funding, with the gap most obvious between the ages of 14 and 18; families were often unsure which activities suited their child, and GPs lacked the information to signpost them. In the survey, 23% of parents and carers strongly disagreed that their child had opportunities to take part in the community and build relationships, while only 15% agreed.
Health produced the most troubling findings, and the report describes a cliff edge in support as young people approach 18. Children's health services often started transition planning too late, mental health services especially, and young people either had no equivalent adult service or could not get into one; parents described the years between 16 and 18 as no man's land. There were strong counter-examples, including an adult learning disability service with a transition nurse who takes referrals from 17 and a half and can offer a service the day a young person turns 18.
A second group gets attention in the report: young people who are known to children's health services but fall below the threshold for adult services. Annual learning disability health checks were not consistently carried out, and some parents did not know a check had happened or what it was for; where a GP prescribes ADHD medication and wants a review, the referral has to go back to a psychiatrist, adding another wait. Inspectors found a transition key worker service that supports young people to 25 and helps families book appointments, but guidance telling adult services to make reasonable adjustments was described by practitioners as unclear about what it means in practice.
Waiting times run through the report, because a late diagnosis pushes back every later step of preparation. Inspectors cite the Children's Commissioner's analysis from October 2024 showing that children can wait on average more than two years for a neurodevelopmental diagnosis in community health services, and some wait four years or more. A case in the report describes a young person who waited almost a year for an autism assessment and could not access vital additional support without a formal diagnosis; NHS England is urged to make sure children referred before the age of 16 receive a timely formal diagnosis so that transition planning can start early.
The report contrasts stronger and weaker partnerships. Where arrangements were stronger, preparation for adulthood began in the early years and was a consistent part of the EHC plan, services were designed as all age provision running to 25, and children in care or care leavers kept consistent workers beyond 18. Weaker partnerships did not jointly commission services, communicated poorly with parents and carers about what was available, shared little data across the partnership, and were slow to start transition to adult health services.
Sixteen recommendations close the report, addressed to the Department for Education, the Department of Health and Social Care, the housing department, NHS England, local area partnerships, local authorities, integrated care systems and boards, and schools and colleges. The most useful for families are the calls for preparation for adulthood to be built into a national EHC plan template, for a transitional period of support when a plan ends, and for local authorities to review preparation for adulthood at every available opportunity, which makes the annual review a point parents can use. The report makes no individual judgements and no area is graded because of a thematic visit; its purpose is to change how the system works.
