Schools rarely see a diagnosis; they see changes in how a student functions. Attendance, homework, participation and friendships are the visible markers, and staff are asked to log the pattern rather than label the cause. Keeping children safe in education 2025 states that only trained professionals should diagnose a mental health problem, while teachers are well placed to notice what behaviour suggests. A workable rule for families: exam nerves usually fade once the paper is over, but sleep loss, appetite change and withdrawal from friends that lasts more than a fortnight, and shows up in more than one setting, deserves a conversation rather than a wait-and-see approach.
Every school in England is required to have a designated safeguarding lead, and paragraph 102 of KCSIE 2025 makes clear that this responsibility belongs in the role holder's job description. Where a mental health concern is also a safeguarding concern, such as self-harm, sustained bullying or a sudden change at home, the guidance says action should follow the child protection policy and reach the DSL or a deputy without delay. Most settings also name a senior mental health lead for the whole-school approach rather than individual cases, and gov.uk guidance allows this to be a headteacher, a deputy or another senior leader. Waiting until something feels serious enough is a common mistake, and by the time attendance collapses the timeline is hard to reconstruct.
第三层才是 NHS 的专科服务 CAMHS(儿童与青少年心理健康服务),需要转介和评估。
Support in England is usually described in three layers. Universal provision covers every student: the classroom climate, mental health education and sensible routines during exam season. Targeted provision sits inside the school and increasingly includes Mental Health Support Teams, run jointly by the NHS and education settings since the 2018/19 academic year and staffed by trained education mental health practitioners. Data published by the Department for Education and NHS England in 2026 puts MHST coverage above 5.8 million pupils and learners in 2025-26, which is 60% of the school and further education population and 79% of pupils in state-funded secondary schools; the 2025 Spending Review confirmed a target of full coverage by 2029/30. The third layer is specialist NHS provision, child and adolescent mental health services, known as CAMHS, which works on referral and assessment. Parents often assume an MHST is the same as a school counsellor. It is closer to early intervention: short, evidence-based, and quick to escalate when the need is beyond its scope.
The route from school to specialist help normally runs through in-school support, then a referral that is often made by a GP, and finally a specialist assessment. Waiting is where the system draws the most criticism. NHS England consulted in July 2021 on a four-week standard for children and young people referred to community mental health services, but its own 2026 data note records that the four-week pilots have ended and the work now sits inside the clinically-led review of standards, so it is not yet an operating children's waiting standard. What is fully in force and monitored is eating disorder provision: 95% of urgent referrals should start treatment within one week and 95% of routine referrals within four weeks. A House of Commons Library briefing published in 2024 shows how far practice trails that benchmark, with 66% of urgent referrals starting treatment within a week and 77% of routine referrals within four weeks between September and November 2023. Those two figures explain why identification at school level carries so much weight.
A joint targeted area inspection examines a local area rather than a single school. The six inspections carried out between September 2019 and February 2020 were led jointly by Ofsted, the Care Quality Commission, HMICFRS and HMI Probation, and they asked whether social care, health, youth justice, schools and the police actually worked together; one deep dive focused on the experiences of children aged 10 to 15, including those on child protection plans and children in care. The report, published on 9 December 2020 under the title Feeling heard, found that areas with effective partnership working had a single point of access for advice and referrals, and children there were identified sooner. Where agencies kept separate records and did not share information, children fell through the gaps between services. The Hillingdon inspection in June 2025 used the same framework and the same four inspectorates, which shows the method is still live.
EHC plan
当心理困难已经影响到学习,SEND Code of Practice 2015(覆盖 0 至 25 岁)把社会、情绪与心理健康困难(SEMH)列为经过承认的需求类别,学校应当按 assess, plan, do, review 的循环先提供 SEN 支持,再评估是否需要 EHC plan,法律基础是 2014 年的 Children and Families Act。
When mental health difficulties begin to affect learning, the SEND Code of Practice 2015, which covers ages 0 to 25, treats social, emotional and mental health as a recognised category of need. Schools are expected to follow the assess, plan, do, review cycle, putting SEN support in place first and only then considering whether an EHC plan is required; the legal basis is the Children and Families Act 2014. The common mistake is reading SEMH as attitude, so a child is recorded as defiant and loses behaviour points instead of being assessed as needing support. Schools can request an EHC needs assessment and parents can ask the local authority directly, and once a request is made statutory timescales apply at each stage. Written records started early make every later conversation easier.
Choosing the right person matters more than finding the perfect words. The form tutor or head of year handles day-to-day changes, the DSL is the right contact where safety or self-harm is involved, and the SENCo deals with learning support. In a meeting, a timeline beats a diagnosis: two late arrivals a week over three weeks and three missed physics deadlines is concrete enough to start a support process, while a general statement that a child seems depressed often is not. Schools usually need written consent from a parent before sharing information with an MHST or CAMHS, so delaying that form delays the referral itself. Once a student turns 16, consent for information sharing normally has to come from the student, and parents no longer automatically see everything, which is why many families find information dries up during A-Level years.
At GCSE and A-Level, families often ask whether a mental health condition can be converted into exam adjustments. In the JCQ regulations for 2025/26, 25% extra time is an access arrangement that must be supported by evidence of need and by the candidate's normal way of working; anything above 25% and up to 50% is granted only in exceptional circumstances and needs a substantial body of centre-based evidence, usually submitted on a form such as Form 8. The scope is worth stating plainly: extra time addresses long-term learning difficulties or disabilities rather than a difficult few weeks before the exam. Where illness or an unforeseen event damages performance during the exam period, JCQ runs a separate special consideration process for the awarding body to weigh during marking. Requests go through the school's exams officer, and thin applications are returned rather than approved. What students can control directly is the routine and the requests they make in the weeks beforehand.
The whole path compresses into one line: problems are first noticed at school, and the next step is decided jointly by the school, the NHS and the family. What families can do immediately is keep a simple timeline of when it started, where it shows up and what it is affecting, then take that record into conversations with the form tutor, the DSL or a GP. Students aged 16 and over can request an NHS referral themselves, without waiting for a parent to agree. In practice, the sooner somebody joins the separate observations into one picture, the less time a child spends moving between services.
