Edexcel ALevel

The immune system is one of the most marks-heavy and discriminating topics in A-Level Biology. Under Edexcel Biology A, the Salters-Nuffield specification, immunity sits inside Topic 6, Infection, Immunity and Forensics, and the exam expects far more than definitions. You must be able to write out the full sequence of phagocytosis, cell-mediated immunity and humoral immunity, and explain precisely how B cells are activated and how plasma cells differ from memory cells. Many students can define an antibody but freeze when asked why a secondary response is faster and stronger. This bilingual guide walks through the first line of defence, inflammation, phagocytosis, the division of labour between T and B cells, antibody structure, vaccines and the mechanism of HIV, then finishes with the highest-yield exam points and a writing template.

Pathogens include bacteria, viruses, fungi and protoctists. The body defends itself at two levels. Non-specific immunity acts against any invader, while specific immunity targets one particular pathogen. The first line of defence is non-specific. The tough, keratinised outer layer of the skin is a physical barrier. In the airways, cilia beat in waves to push mucus, together with trapped microbes, up towards the throat, where it is swallowed or coughed out. Hydrochloric acid in the stomach kills most bacteria that arrive with food, while lysozyme in tears and saliva digests bacterial cell walls. Because none of these mechanisms is aimed at one named pathogen, the response is described as non-specific. A favourite Edexcel question gives a scenario and asks which defence is involved, for example inhaled dust trapped in airway mucus. The expected answer is that cilia and mucus remove the microbes, not a vague reference to the skin.

When pathogens break through the first line, tissue damage triggers the inflammatory response. Damaged mast cells release histamine, which makes capillaries dilate and become more permeable. More blood flows to the area, and plasma proteins and fluid leak into the tissue, producing the classic signs of redness, swelling, heat and pain. Neutrophils from the blood, and macrophages that develop from monocytes, are then attracted to the infected site by chemical signals, a movement called chemotaxis. Phagocytosis must be written as a fixed sequence for full marks. First, the phagocyte recognises molecules on the pathogen surface using its membrane receptors. Second, the membrane folds inwards, engulfing the pathogen to form a phagosome. Third, lysosomes fuse with the phagosome and release hydrolytic enzymes that digest the pathogen. Fourth, a macrophage displays antigen fragments on its cell surface membrane and becomes an antigen-presenting cell. Examiners award marks for the key verbs fuse, hydrolyse and present, so never stop your answer at engulf, which earns only half credit.

Specific immunity splits into two pathways with different jobs. Cell-mediated immunity deals with the body's own cells that have already been infected. A macrophage presents the antigen to T helper cells, whose surface receptors bind to it. Activated T helper cells divide rapidly and release cytokines, signalling molecules that activate cytotoxic T cells. Cytotoxic T cells kill by releasing proteins such as perforin, which punch holes in the membrane of the infected cell. The cell lyses and dies, the pathogens inside are exposed, and phagocytes clear them away. A crucial concept is that T cells never detect free-floating pathogens directly. They only recognise antigens presented on the surface of body cells by MHC molecules. This also explains transplant rejection, the recipient's cytotoxic T cells treat donor cells as foreign and attack them. In short-answer questions, keep the division of labour clear: infected body cells are handled by cell-mediated immunity, while free pathogens in blood and tissue fluid are handled by humoral immunity. Swap the two pathways and you lose the whole question.

The key players of humoral immunity are B lymphocytes. Every B cell carries antibody receptors on its surface, and each receptor binds only one shape of antigen. Binding alone is not enough. The B cell must also receive cytokines released by an activated T helper cell as a second signal. This two-signal requirement prevents the immune system from attacking the body's own tissues by mistake. Once fully activated, the B cell undergoes clonal selection and clonal expansion: it divides repeatedly to form a large population of identical cells, which then differentiate. Plasma cells are antibody factories, each one secreting thousands of antibodies per second, but they are short-lived. Memory cells can survive for decades; when the same antigen appears again they divide rapidly into plasma cells, which immediately produce large amounts of antibody. This is why the primary response is slow and weak while the secondary response is fast and strong. During the secondary response the antibody concentration rises higher and peaks sooner, and memory cells usually clear the pathogen before symptoms even appear.

Antibodies, also called immunoglobulins, are Y-shaped proteins secreted by plasma cells. Each antibody molecule contains four polypeptide chains, two identical heavy chains and two identical light chains, joined by disulfide bonds. The two tips of the Y form the variable region, where the amino acid sequence differs between antibodies and creates the antigen-binding site. The shape of this site is complementary to one specific antigen, which is why each antibody is specific. The base of the molecule is the constant region, whose sequence barely changes between antibodies. When the variable region binds an antigen, an antigen-antibody complex forms. Antibodies do not destroy pathogens directly. Instead they help in three main ways. Neutralisation: an antibody coats a virus or a toxin so that it can no longer attach to host cells. Agglutination: one antibody can bind two or more pathogens, clumping them together so that a single phagocyte can engulf many at once. Opsonisation: a pathogen coated with antibodies is recognised and engulfed by phagocytes much more easily. For a six-mark antibody question, structure your answer as Y shape, four chains, variable region for specific binding, then one clearance mechanism explained in detail, and you collect both the structure marks and the function marks.

A vaccine is artificial active immunity. A killed or weakened pathogen, or a purified antigen, is injected so that the immune system produces memory cells without the person suffering the disease. The antigen is presented by antigen-presenting cells, T helper and B cells are activated, and the primary response generates both antibodies and memory cells. When the real pathogen later arrives, memory cells trigger a rapid secondary response and the person stays healthy. Herd immunity describes the situation where a high enough proportion of the population is vaccinated that the pathogen cannot easily spread between people, which protects individuals who cannot be vaccinated because of age, illness or allergy. Two vaccine questions appear again and again in Edexcel papers. First, why must the flu vaccine be reformulated every year? Because influenza viruses change their surface antigens constantly through antigenic variation, and last year's memory cells do not recognise this year's strain. Second, why is a vaccine not effective for every individual? Possible reasons include differences in the person's immune response, immunological memory declining over time, and mutation of the pathogen.

HIV, the human immunodeficiency virus, attacks the immune system itself. Two ideas unlock the whole topic: reverse transcriptase and CD4. HIV is a retrovirus, so its genetic material is RNA, and its outer coat carries a glycoprotein called gp120. It mainly infects T helper cells because these cells display CD4 receptors on their surface, and gp120 binds to CD4. Once inside, reverse transcriptase copies the viral RNA into DNA, and this DNA is inserted into the chromosomes of the host cell, forming a provirus that is replicated together with the host DNA. Infected T helper cells die in large numbers, so the helper cell count falls steadily and both cell-mediated and humoral immunity collapse. When the CD4 count drops to a very low level, the patient suffers opportunistic infections such as tuberculosis and Pneumocystis pneumonia, and this stage is called AIDS. A key contrast with an ordinary cold is that early HIV infection may cause few or no symptoms, and the latent period can last for years, during which the person can still transmit the virus. In exams, never write that HIV kills the person directly, the actual cause of death is complications from secondary infection.

Condense the whole topic into this revision checklist. First, be able to write out the five steps of phagocytosis from memory: recognition, engulfing, phagosome formation, lysosome fusion and antigen presentation. Second, keep the two lymphocytes separate in your head. T cells mature in the thymus and run cell-mediated immunity, attacking infected cells directly. B cells run humoral immunity and differentiate into plasma cells and memory cells. Third, the graph comparing primary and secondary responses is an almost guaranteed question, and both the height gap and the time gap between the two curves come from memory cells. Fourth, for antibody questions, describe structure first and then function: the variable region binds a specific antigen, and the constant region supports clearance. Fifth, vaccine questions always expand around memory cells plus the secondary response. Sixth, HIV answers must mention reverse transcriptase and T helper cells. Finally, watch your spelling, because examiners deduct marks for misspelling terms such as plasma cell, memory cell and clonal expansion. Before the exam, copy each English keyword from this article three times and then explain the topic aloud in Chinese to a classmate. Active recall beats re-reading notes every time.
