A-Level AQA
一、心理病理学在AQA考纲中的位置 | Where Psychopathology Sits in the AQA Specification
Psychopathology is one of the four major topics in AQA A-Level Psychology Paper 1, alongside Memory, Attachment and Social Influence, and it typically accounts for a substantial share of the marks. This module studies the criteria used to judge abnormality, and uses three disorders, phobias, depression and OCD, to show how the behavioural, cognitive and biological approaches each explain and treat psychological disorders. Note that AQA places Psychopathology in Paper 1, which means it forms the foundation of the exam together with Research Methods and Biopsychology. What you need to master is not only the characteristics of the three disorders, but also the ability to compare the logic behind each approach's explanation and treatment, which is a frequent starting point for essay questions. This article follows the specification order, beginning with definitions of abnormality and then unpacking the three disorders one by one.
The Four Definitions of Abnormality
The AQA specification requires you to know four definitions of abnormality. The first is statistical infrequency, where behaviour is judged more abnormal the rarer it is, for example an extremely low or high IQ is statistically rare. The second is deviation from social norms, where behaviour violates unwritten rules of society, such as people with antisocial personality disorder who often disregard others' feelings. The third is failure to function adequately, proposed by Rosenhan and Seligman, which includes specific criteria such as maladaptiveness, personal distress, unpredictability, irrationality and observer discomfort. The fourth is deviation from ideal mental health, proposed by Jahoda, which lists positive criteria including a positive self-attitude, self-actualisation, autonomy, resistance to stress, accurate perception of reality and mastery of the environment. These four definitions each have a different emphasis, and understanding their differences is key to answering definition questions.
Strengths and Limitations of Each Definition
Statistical infrequency is objective and quantifiable, drawing a clear boundary between normal and abnormal, but it labels rare yet positive traits such as high intelligence as abnormal, and the choice of cut-off point is arbitrary. Deviation from social norms can distinguish between behaviours that meet or violate expectations, but it depends heavily on culture and era, for example homosexuality was once classed as a mental disorder, which is no longer the case. The failure to function criterion respects the individual's subjective experience and acknowledges that distress itself deserves attention, but some dysfunctional behaviour can also be adaptive, such as pre-exam anxiety that motivates revision. Deviation from ideal mental health is positive and holistic, focusing on personal growth rather than simply labelling, but Jahoda's criteria are too demanding, as most people cannot fully meet them at certain periods. In evaluation questions, you should offer the specific evaluation that matches each definition rather than saying vaguely that all definitions are flawed.
Phobias: Characteristics and the Two-Process Model
A phobia is an extreme and irrational fear of a specific object or situation. Its behavioural characteristics are panic and avoidance, for example someone with arachnophobia may scream and run out of a room; its emotional characteristics are intense anxiety and fear; and its cognitive characteristics are irrational beliefs and selective attention, where the person overestimates the threat and focuses only on the feared stimulus. Phobias are explained by Mowrer's two-process model. In the first stage, fear is acquired through classical conditioning, as shown in the famous Little Albert experiment by Watson and Rayner, where a white rat was repeatedly paired with a loud noise until Albert came to fear the rat itself. In the second stage, the fear is maintained through operant conditioning, because avoidance reduces anxiety and is therefore negatively reinforced, so the person keeps escaping and the fear never extinguishes.
Systematic Desensitisation and Flooding
The behavioural approach treats phobias using two main methods. Systematic desensitisation, proposed by Wolpe, is based on reciprocal inhibition, the idea that a person cannot be relaxed and anxious at the same time. Treatment involves three steps: first training deep relaxation, then working with the patient to build an anxiety hierarchy from the mildest to the most feared stimulus, and finally having the patient gradually imagine or face the stimuli while relaxed, so relaxation replaces anxiety through counterconditioning. Flooding instead exposes the patient to the most intense feared stimulus all at once, without any gradual build-up, allowing the fear response to weaken through extinction in the absence of real harm. Systematic desensitisation is gentle and progressive, suiting most patients, while flooding works quickly but is intense, may cause significant distress, and also raises ethical issues such as informed consent.
Depression: Characteristics and Beck's Negative Triad
Depression is a mood disorder whose characteristics are also divided into three categories. The behavioural characteristics are reduced activity levels, where the person may struggle to get out of bed and avoid social contact; the emotional characteristics are persistently low mood, irritability and low self-esteem; and the cognitive characteristics are poor concentration, difficulty making decisions and persistent negative thinking. Depression is explained by the cognitive approach. Beck proposed the negative triad, arguing that people with depression hold negative views of the self, the world and the future, for example believing they are worthless, the world is hopeless and the future will only get worse. These negative views come from negative schemas formed by early experience. Once activated by life events, they lead the person to magnify negative information and ignore positive information through faulty information processing such as selective abstraction and overgeneralisation, thereby maintaining the depression.
七、埃利斯ABC模型与认知行为疗法 | Ellis's ABC Model and Cognitive Behavioural Therapy
Ellis extended the cognitive explanation with the ABC model: A is the Activating event, B is the Belief, and C is the Consequence. He argued that emotional distress comes not from the event itself but from the individual's irrational beliefs about it, for example musturbatory thinking such as "I must be liked by everyone" can turn an ordinary rejection into a complete breakdown. Depression is treated with cognitive behavioural therapy, which combines cognitive restructuring with behavioural change. Beck's form of CBT helps the patient identify and challenge negative thoughts, testing them against real evidence, while Ellis's rational emotive behaviour therapy directly disputes the irrational beliefs. Behavioural activation is also added to encourage the patient to re-engage with activities that bring a sense of achievement. Research shows CBT is as effective as medication and has a lower relapse rate, providing strong empirical support for the cognitive explanation.
OCD: Characteristics and Biological Explanations
OCD is characterised by obsessions and compulsions. The behavioural characteristics are repetitive ritualistic actions such as repeated hand washing or checking door locks; the emotional characteristics are intense anxiety and distress, because the person knows the compulsions are irrational yet cannot stop them; and the cognitive characteristics are persistent intrusive thoughts, with most patients retaining insight and knowing these thoughts are unnecessary. OCD is explained by the biological approach. Genetically, twin and family studies such as Nestadt's show that OCD has a heritable component, linked to the COMT gene and the SERT gene, and follows the diathesis-stress model, where genetic vulnerability needs an environmental stressor to be triggered. Neurally, research finds abnormal serotonin levels and overactivity in the orbitofrontal cortex and basal ganglia, forming a worry circuit that makes it hard for the person to inhibit repetitive behaviour.
九、SSRI药物治疗与评估 | SSRI Drug Therapy and Evaluation
The biological treatment for OCD is drug therapy, with SSRIs, selective serotonin reuptake inhibitors, as the first choice, for example fluoxetine. SSRIs work by blocking the reuptake of serotonin by the presynaptic neuron, raising the concentration of serotonin in the synaptic gap and thereby reducing symptoms. If SSRIs are ineffective, other drugs are used in combination, such as the tricyclic antidepressant clomipramine or SNRIs, and they are often combined with CBT to boost the effect. A meta-analysis by Soomro and colleagues confirmed that SSRIs are significantly better than placebo, so their effectiveness is well documented. However, drug therapy has limitations: it takes weeks to work, has side effects such as nausea and insomnia, is prone to relapse after stopping, and it only relieves symptoms without addressing the psychological cause of OCD. Evaluation questions in the exam often ask you to discuss both the effectiveness and these limitations of biological treatments.
Exam Technique and Common Mistakes
Essay questions in Psychopathology usually ask you to "describe and evaluate" an explanation or therapy, so you must separate the AO1 knowledge from the AO3 evaluation, first stating the core content of the model or therapy accurately, then evaluating it with named studies, alternative explanations and limitations. Common mistakes include confusing the two stages of the two-process model by writing the maintenance stage as classical conditioning, mixing up the reciprocal inhibition principle of systematic desensitisation with the extinction principle of flooding, forgetting to mention the high IQ example when evaluating statistical infrequency, and mixing up Beck's negative triad of self, world and future with Ellis's ABC of event, belief and consequence. When answering, try to cite researchers' names and findings, such as Little Albert, Nestadt's twin studies and Soomro's meta-analysis, as these directly raise the professionalism of your answer. Finally, when comparing approaches, keep the three explanation and treatment strands of behaviourism, cognition and biology correctly matched, and do not mix them up.
