AQA A-Level
Psychopathology is one of the three core modules in AQA A-Level Psychology Paper 1, alongside Memory and Attachment. Students must master four definitions of abnormality, compare the behavioural, emotional, and cognitive characteristics of phobias, depression, and OCD, and understand the explanation-and-treatment approaches for each disorder. Typically, Psychopathology accounts for two short-answer questions and one 16-mark essay in the exam - a critical module for achieving A-A* grades.
The AQA specification requires four abnormality definitions. Statistical Infrequency uses the normal distribution, defining traits beyond two standard deviations as abnormal - objective and quantifiable, but ignores that some rare behaviours are positive (e.g., high IQ). Deviation from Social Norms relies on culturally accepted standards, offering contextual sensitivity but suffering from historical and cultural relativism (e.g., homosexuality was once classified as a disorder). Failure to Function Adequately, proposed by Rosenhan and Seligman, assesses impairment in daily functioning through seven criteria including personal distress and observer discomfort - it captures subjective experience but may pathologize reasonable responses to adverse environments. Deviation from Ideal Mental Health, proposed by Jahoda, lists six positive criteria such as self-actualisation and resistance to stress - comprehensive and positive, but the criteria are so demanding that few individuals would meet them all.
Phobias require three symptom categories for AQA: behavioural (panic responses, avoidance, endurance), emotional (excessive and persistent anxiety that is disproportionate to the threat), and cognitive (irrational beliefs about the phobic stimulus, selective attention, cognitive distortions). Mowrer's Two-Process Model explains phobia acquisition through classical conditioning (a neutral stimulus paired with a traumatic event triggers fear) and maintenance through operant conditioning (avoidance behaviour is negatively reinforced by anxiety reduction). Watson and Rayner's Little Albert experiment (1920) provides classical empirical support, though it has a single-participant limitation and ethical concerns.
Two behavioural treatments are specified. Systematic Desensitisation (SD), developed by Wolpe (1958), involves three stages: learning progressive muscle relaxation, constructing an anxiety hierarchy with the therapist from least to most frightening scenarios, and graduated exposure while in deep relaxation. Its core mechanism is reciprocal inhibition - relaxation and anxiety cannot coexist. Gilroy et al. (2003) followed 42 spider-phobic patients treated with SD and found the treatment group showed significantly lower symptoms at a 33-month follow-up compared to a relaxation-only control group. SD is suitable for all ages and has low dropout rates, though treatment duration is longer. Flooding exposes patients immediately to their most feared situation until anxiety subsides naturally - extremely fast (often one to two sessions) but with high dropout rates and significant ethical concerns due to intense initial distress.
Depression also requires three symptom categories: behavioural (reduced activity levels, disrupted sleep and appetite, aggression and self-harm), emotional (persistent low mood, anhedonia, low self-esteem often accompanied by anger), and cognitive (persistent negative thinking, reduced concentration, absolutist thinking, suicidal ideation). Beck's Negative Triad proposes that depressed individuals hold systematic negative schemas about the self, the world, and the future. These schemas typically develop in childhood through experiences such as parental criticism and peer rejection and, when activated in adulthood by similar situations, generate automatic negative thoughts that evolve into cognitive biases like overgeneralisation and catastrophising. Ellis's ABC Model argues that Activating Events do not directly cause depression; rather, irrational Beliefs - such as "I must be liked by everyone" or "I must be perfect at everything" - produce the emotional and behavioural Consequences.
CBT is the core treatment for depression in the AQA specification. Cognitive Behavioural Therapy integrates Beck's and Ellis's theories, involving: first, identifying and challenging automatic negative thoughts through cognitive restructuring; second, encouraging patients to re-engage in activities that provide a sense of achievement or pleasure through behavioural activation, breaking the vicious cycle of inactivity leading to worsening mood. Ellis's Rational Emotive Behaviour Therapy (REBT) is a CBT variant using the DEF steps - Dispute irrational beliefs, produce Effective new philosophies, and achieve healthier Feelings - with the therapist playing a more directive role. March et al. (2004) conducted an RCT with 327 adolescents and found CBT alone was effective in 81% of cases, CBT combined with fluoxetine was 86% effective, compared to only 35% in the placebo group. CBT is strongly evidence-based and free of medication side effects, but requires patients to have sufficient cognitive ability and motivation, and offers less immediate relief than medication for severe depression.
OCD's biological explanations are frequently examined in AQA Paper 1 essays. Genetic explanations rest on two key findings: Nestadt et al.'s (2000) meta-analysis found a 12% first-degree relative prevalence rate for OCD compared to 3% in controls, indicating familial clustering; twin studies show a 68% concordance rate for MZ twins versus 31% for DZ twins, strongly implicating genetic factors. Candidate genes include COMT and SERT gene variants, but OCD is understood to be polygenic - individual gene effects are small. Neural explanations focus on the frontal-striatal-thalamic loop, whose hyperactivity is linked to compulsive behaviours; PET scans reveal abnormally elevated metabolic rates in the orbitofrontal cortex (OFC) of OCD patients, and serotonin-dopamine system imbalances are associated with clinical symptoms.
OCD的标准药物治疗是SSRIs(选择性5-羟色胺再摄取抑制剂),如氟西汀(Prozac)。
The standard pharmacological treatment for OCD is SSRIs (Selective Serotonin Reuptake Inhibitors), such as fluoxetine (Prozac). SSRIs block serotonin reuptake at the presynaptic neuron, increasing serotonin concentration in the synaptic cleft and reducing obsessive-compulsive symptoms. Soomro et al.'s (2009) Cochrane review of 17 RCTs confirmed SSRIs are significantly more effective than placebo for OCD, with a moderate effect size. For SSRI-resistant patients, augmentation with tricyclic antidepressants (e.g., clomipramine) or atypical antipsychotics may be considered. Advantages include strong empirical support, ease of administration, and minimal cognitive demands on patients. Limitations include side effects (nausea, headache, sexual dysfunction) causing premature discontinuation, symptom improvement being temporary with high relapse rates after cessation, and biological reductionism ignoring psychosocial factors like environmental stress in OCD onset.
The 16-mark essay in Psychopathology follows a "describe plus evaluate" structure, with AQA allocating 6 marks for AO1 (knowledge) and 10 marks for AO3 (evaluation). Key strategies for top grades: research evidence must include precise data (e.g., Gilroy 2003's 33-month follow-up, March 2004's 81% efficacy rate), not just vague claims of empirical support; evaluation should follow the PEEL structure (Point, Evidence, Explain, Link) with each paragraph focusing on one evaluative point; the biological, behavioural, and cognitive paradigms must be compared - cover at least two; conclusions should be integrative, such as "overall, the biopsychosocial model may provide the most comprehensive understanding." Note AQA's definition of "effective discussion": listing studies is not discussion - you must demonstrate critical analysis including methodological limitations, alternative explanations, and real-world application implications.
