GCSE Edexcel Biology — Health and Disease: A Complete Guide
Health is one of the most important topics in GCSE Edexcel Biology, appearing in both Paper 1 and Paper 2. This article covers everything you need to know: from the World Health Organisation's definition of health to communicable diseases, the immune system, and lifestyle-related non-communicable diseases.
The World Health Organisation (WHO) defines health as "a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity." This definition is crucial for your exam — examiners love to ask about it. Notice that being "healthy" is not just about not being sick; it encompasses physical fitness, mental health, and social well-being.
Diseases are broadly classified into two categories: communicable diseases and non-communicable diseases.
Communicable diseases are caused by pathogens — microorganisms that can spread from one person to another. The four main types of pathogens tested in GCSE Edexcel Biology are: viruses, bacteria, fungi, and protists.
Viruses are not living cells. They are much smaller than bacteria and reproduce by invading host cells and hijacking their machinery to produce copies of themselves. Key viral diseases you must know include Measles, HIV, and Tobacco Mosaic Virus (TMV).
Measles is a viral disease spread by droplets from coughs and sneezes. Symptoms include fever and a red skin rash. It can be serious, leading to pneumonia or encephalitis. Most children are vaccinated against measles with the MMR vaccine.
HIV (Human Immunodeficiency Virus) is spread by sexual contact or exchange of bodily fluids such as blood. It attacks the immune system's white blood cells, eventually leading to AIDS — where the body becomes susceptible to opportunistic infections. Antiretroviral drugs can control HIV but currently there is no cure.
HIV(人类免疫缺陷病毒)通过性接触或血液等体液交换传播。
Tobacco Mosaic Virus (TMV) affects plants, causing a mosaic pattern of discolouration on leaves, which reduces photosynthesis and crop yield. It spreads through contact between plants and contaminated tools.
Bacteria are single-celled living organisms. Unlike viruses, they can reproduce on their own. Some bacteria are harmful, but many are beneficial. Key bacterial diseases include Salmonella and Gonorrhoea.
Salmonella causes food poisoning. It is spread by eating contaminated food, especially undercooked poultry and eggs. Symptoms include fever, abdominal cramps, vomiting, and diarrhoea. In the UK, chickens are vaccinated against Salmonella to control its spread.
Gonorrhoea is a sexually transmitted disease (STD) caused by bacteria. Symptoms include thick yellow or green discharge from the genitals and pain when urinating. It is treated with antibiotics, but antibiotic-resistant strains are emerging.
Fungal diseases in plants include Rose Black Spot, which causes purple or black spots on rose leaves. The leaves turn yellow and drop early, reducing photosynthesis. It spreads through water or wind and is treated with fungicides or by removing and destroying affected leaves.
Protists are single-celled eukaryotic organisms. The key protist disease for GCSE is Malaria, caused by Plasmodium parasites. Malaria is spread by female Anopheles mosquitoes, which act as vectors. Symptoms include recurrent fevers and shaking. Prevention methods include mosquito nets, insect repellent, and antimalarial drugs.
Now let's talk about how the body defends itself — the immune system. The human body has several lines of defence against pathogens.
The first line of defence is physical and chemical barriers. The skin acts as a physical barrier, and if it is cut, platelets in the blood form a clot to seal the wound. The respiratory system has mucus and cilia that trap and sweep away pathogens. The stomach produces hydrochloric acid that kills pathogens in food.
The second line of defence is the non-specific immune response. White blood cells called phagocytes engulf and digest pathogens — a process called phagocytosis.
The third line of defence is the specific immune response involving lymphocytes. When a pathogen enters the body, lymphocytes recognise the antigens on its surface and produce specific antibodies. Antibodies bind to antigens, clumping pathogens together so phagocytes can destroy them more efficiently. Some lymphocytes become memory cells, which remain in the body and provide long-term immunity — this is why you don't usually get the same disease twice.
Vaccination exploits this mechanism. A vaccine contains dead or weakened pathogens that carry antigens. These trigger the immune system to produce antibodies and memory cells without causing illness. If the person is later exposed to the real pathogen, memory
cells rapidly produce antibodies, preventing infection. Herd immunity occurs when a large proportion of the population is vaccinated, reducing the spread of the disease and protecting those who cannot be vaccinated.
Antibiotics are drugs that kill bacteria or prevent their growth. They do NOT work against viruses. This is a very common exam question — make sure you know the difference! Painkillers such as paracetamol relieve symptoms but do not kill pathogens.
Antibiotic resistance is a growing global concern. Bacteria can mutate, and some mutations make them resistant to antibiotics. When antibiotics are used, non-resistant bacteria die, but resistant ones survive and reproduce, passing on the resistance gene. This is natural selection in action. To slow the development of antibiotic resistance, doctors should only prescribe antibiotics when necessary, and patients must complete the full course of antibiotics.
Non-communicable diseases are not caused by pathogens and cannot be transmitted between people. They include cardiovascular disease, cancer, type 2 diabetes, and respiratory diseases. Many non-communicable diseases are linked to lifestyle factors.
Risk factors for non-communicable diseases include smoking, poor diet, lack of exercise, obesity, and excessive alcohol consumption. Smoking damages the lungs and increases the risk of lung cancer, emphysema, and cardiovascular disease. A diet high in saturated fat and cholesterol can lead to fatty deposits in arteries, increasing the risk of coronary heart disease. Lack of exercise and obesity are linked to type 2 diabetes and cardiovascular problems.
Coronary heart disease occurs when the coronary arteries, which supply the heart muscle with oxygenated blood, become narrowed by fatty deposits called plaques. This reduces blood flow to the heart, which can lead to angina or a heart attack. Treatments include statins (which lower cholesterol), stents (which keep arteries open), and in severe cases, bypass surgery or a heart transplant. A faulty heart valve can be replaced with a biological or mechanical valve.
Cancer is the result of uncontrolled cell division, forming a tumour. Benign tumours are not cancerous — they grow slowly, stay in one place, and do not invade other tissues. Malignant tumours are cancerous — they grow rapidly, invade neighbouring tissues, and can spread to other parts of the body via the blood, forming secondary tumours. This process is called metastasis. Risk factors for cancer include genetic predisposition, smoking, UV exposure, and certain viral infections.
Plants also get diseases and have defences. Plant diseases can be caused by viruses (TMV), bacteria, fungi (Rose Black Spot), or environmental factors like mineral deficiencies. Nitrate deficiency causes stunted growth because nitrates are needed for protein synthesis. Magnesium deficiency causes chlorosis (yellowing of leaves) because magnesium is needed to make chlorophyll.
Plants have physical defences including the waxy cuticle, cell walls, and layers of dead cells like bark. They also have chemical defences, such as producing antibacterial compounds and poisons to deter herbivores. Mechanical defences include thorns, hairs, and leaves that droop or curl when touched (like Mimosa pudica).
Let's summarise the key points for your exam revision. Health is a state of complete physical, mental, and social well-being. Pathogens include viruses, bacteria, fungi, and protists. The body defends itself through physical barriers, phagocytosis, and antibody production. Vaccination creates immunity without causing disease. Antibiotics kill bacteria but not viruses, and antibiotic resistance is a major concern. Lifestyle factors such as smoking, poor diet, and lack of exercise increase the risk of non-communicable diseases including coronary heart disease and cancer. Plants also face diseases and have evolved physical, chemical, and mechanical defences.
For the GCSE Edexcel Biology exam, make sure you can compare communicable and non-communicable diseases, describe the body's defence systems in sequence, explain how vaccination works with reference to memory cells, and evaluate the benefits and risks of different treatments like statins and stents. Practice drawing and labelling diagrams of the immune response — this is a common 6-mark question.
