What is Disease?
Disease is any condition that impairs the normal functioning of an organism. In OCR A-Level Biology, understanding disease requires you to distinguish between communicable (infectious) and non-communicable diseases, and to appreciate how pathogens cause illness at the cellular and molecular level. Communicable diseases are caused by pathogens such as bacteria, viruses, fungi, and protoctista, and can be transmitted from one organism to another. Non-communicable diseases, including cancer, cardiovascular disease, and genetic disorders, are not infectious — but they may be influenced by lifestyle, environment, or inherited mutations.
Pathogens are microorganisms that cause disease. The OCR specification requires detailed knowledge of four major groups: Bacteria are prokaryotic cells that lack membrane-bound organelles. They reproduce rapidly by binary fission and cause disease by releasing toxins — either endotoxins (released when the bacterial cell breaks down) or exotoxins (secreted during bacterial growth). For example, Mycobacterium tuberculosis causes tuberculosis (TB) by infecting lung tissue and triggering an inflammatory response that damages alveoli. Viruses are non-living infectious agents consisting of genetic material (DNA or RNA) enclosed in a protein capsid. They cannot reproduce independently — they must invade a host cell and hijack its machinery to replicate. The human immunodeficiency virus (HIV) targets helper T cells of the immune system, progressively destroying the body's ability to fight off infection. Fungi are eukaryotic organisms that include yeasts and moulds. Pathogenic fungi cause disease through direct invasion of tissues or by producing mycotoxins. In plants, Phytophthora infestans (an oomycete, technically fungus-like) causes potato late blight — the pathogen that triggered the Irish Potato Famine. Protoctista are single-celled eukaryotic organisms that include parasites such as Plasmodium, the causative agent of malaria. These parasites have complex life cycles involving multiple hosts. Modes of Transmission 传播方式
Understanding how pathogens spread is essential for designing effective control measures. The OCR syllabus identifies three primary transmission routes. Direct transmission occurs when a pathogen is passed from an infected individual to a susceptible host without an intermediate agent. This includes direct contact (touching, kissing, sexual contact), droplet infection (coughing, sneezing), and transplacental transmission (from mother to foetus across the placenta). Tuberculosis spreads via airborne droplets when an infected person coughs, while HIV can be transmitted through sexual contact or from mother to child during pregnancy. Indirect transmission involves an intermediate surface or substance. Fomites are inanimate objects such as bedding, towels, or surgical instruments that can harbour pathogens. Contaminated food and water are common vehicles for pathogens like Vibrio cholerae (cholera) or Salmonella. The spread of malaria provides a classic example of vector transmission: female Anopheles mosquitoes act as biological vectors, carrying Plasmodium parasites from one human host to another during blood meals.
Plants face constant threats from pathogens, and the OCR specification highlights several key examples. Tobacco Mosaic Virus (TMV) infects tobacco and many other plant species, causing mottling and discolouration of leaves. This reduces the plant's photosynthetic capacity, stunting growth and lowering crop yields. TMV spreads through direct contact between plants or via contaminated tools and hands of agricultural workers. Potato late blight, caused by Phytophthora infestans, is one of the most destructive plant diseases in history. The pathogen produces spores that are dispersed by wind and water, germinating on moist leaf surfaces. The hyphae penetrate leaf tissue through stomata, destroying cells and causing the characteristic blackened, rotting appearance. In humid conditions, the disease can destroy an entire field within days. Black Sigatoka, caused by the fungus Mycosphaerella fijiensis, is the most serious disease affecting banana and plantain crops worldwide. The pathogen causes leaf spots that expand and coalesce, reducing the photosynthetic area dramatically — yield losses can reach 50%. Fungicides are widely used for control, but resistance is an increasing problem. Animal and Human Diseases
Tuberculosis (TB), caused by Mycobacterium tuberculosis, remains one of the world's deadliest infectious diseases. The bacteria are inhaled into the lungs, where they are engulfed by alveolar macrophages. However, the bacteria can survive and multiply within these immune cells, forming granulomas — walled-off clusters of infected macrophages surrounded by lymphocytes. Symptoms include persistent cough, chest pain, night sweats, and weight loss. The BCG vaccine provides partial protection, and treatment requires a prolonged course of multiple antibiotics. HIV/AIDS is caused by the human immunodeficiency virus, which specifically targets CD4+ helper T cells. By binding to CD4 receptors and co-receptors on the cell surface, HIV fuses with the cell membrane and releases its RNA into the cytoplasm. Reverse transcriptase converts viral RNA into DNA, which is integrated into the host genome. As the virus replicates, it destroys helper T cells, crippling the immune system. When the CD4 count falls below a critical threshold, the patient develops acquired immunodeficiency syndrome (AIDS), becoming vulnerable to opportunistic infections. Malaria, caused by Plasmodium species and transmitted by Anopheles mosquitoes, has a complex life cycle. When an infected mosquito bites, sporozoites travel to the liver, invade hepatocytes, and multiply. The resulting merozoites are released into the bloodstream, where they infect red blood cells, causing them to rupture in synchronised cycles — producing the characteristic periodic fevers. The Immune System: Lines of Defence
The body's defence against pathogens operates on multiple levels. Primary defences are physical and chemical barriers that prevent pathogen entry — the skin, with its tough keratinised outer layer, acts as a mechanical barrier; mucous membranes in the respiratory, digestive, and reproductive tracts trap pathogens; lysozyme in tears and saliva breaks down bacterial cell walls; stomach acid destroys ingested pathogens. When pathogens breach these barriers, secondary non-specific defences activate. Inflammation increases blood flow to the site of infection, bringing more phagocytes. Mast cells release histamine, causing vasodilation and increased capillary permeability — swelling, redness, and warmth are classic signs. Phagocytosis involves neutrophils and macrophages engulfing pathogens: the phagocyte extends pseudopodia around the pathogen, encloses it in a phagosome, and fuses this with lysosomes containing hydrolytic enzymes to digest the pathogen. The specific immune response — the tertiary defence — is the most sophisticated. Antigen-presenting cells display pathogen antigens on MHC molecules, activating specific helper T cells. These in turn activate B cells, which differentiate into plasma cells producing antibodies, and cytotoxic T cells, which destroy infected host cells. Memory cells remain after infection, providing long-term immunity.
Antibodies are Y-shaped glycoproteins produced by plasma cells. Each antibody has a specific antigen-binding site that is complementary to a particular antigen. The mechanisms of antibody action include agglutination (clumping pathogens together for easier phagocytosis), neutralisation (blocking pathogen binding sites), opsonisation (marking pathogens for phagocyte recognition), and complement activation (triggering the complement cascade to lyse pathogens). Vaccination exploits the immune system's capacity for memory. By introducing antigens in a safe form — attenuated pathogens, inactivated toxins, or recombinant proteins — vaccines stimulate a primary immune response that produces memory cells without causing disease. Upon subsequent exposure to the real pathogen, the secondary response is faster, stronger, and more sustained. Herd immunity occurs when a sufficiently high proportion of the population is vaccinated, breaking the chain of transmission and protecting those who cannot be vaccinated.
Antibiotics are chemical compounds that kill bacteria or inhibit their growth. They work through various mechanisms: inhibiting cell wall synthesis (penicillin blocks transpeptidase enzymes), disrupting cell membrane function, inhibiting protein synthesis (tetracyclines bind to bacterial ribosomes), or interfering with nucleic acid synthesis. Crucially, antibiotics are ineffective against viruses — they target bacterial-specific structures and processes that viruses do not possess. Antibiotic resistance is a growing global crisis. Resistance arises through random mutation or horizontal gene transfer via plasmids. When a population of bacteria is exposed to an antibiotic, susceptible cells die, but any resistant cells survive and reproduce — this is natural selection in action. The surviving population inherits the resistance genes, and over time the antibiotic becomes ineffective. MRSA (methicillin-resistant Staphylococcus aureus) is a prominent example. Responsible antibiotic use — completing prescribed courses and not using antibiotics for viral infections — is essential to slow resistance development. Key Exam Tips and Common Pitfalls
For OCR A-Level Biology exam success, keep these points in mind. Always distinguish between specific and non-specific immunity — phagocytosis is non-specific, while antibody production is specific. When describing the immune response, use precise terminology: "engulf" not "eat", "lysozyme" not "dissolve". Know how to label a phagocyte diagram showing phagosome and phagolysosome formation. A common mistake is confusing the roles of different immune cells — macrophages present antigens, helper T cells activate B cells, plasma cells secrete antibodies, and cytotoxic T cells kill infected cells. Do not mix these up. When discussing vaccination, always mention memory cells — marks are routinely awarded for linking the secondary response to immunological memory. Ensure you can apply concepts of natural selection to antibiotic resistance: variation exists in bacterial populations, antibiotic exposure creates a selection pressure, resistant individuals survive and reproduce, and the frequency of resistance alleles increases over generations. Use the phrase "selection pressure" explicitly.
Master this topic by creating comparison tables for the four pathogen types, drawing annotated diagrams of phagocytosis and the specific immune response, and writing out the malaria life cycle from memory. Practice past paper questions on disease transmission and antibiotic resistance — these are high-frequency exam topics. Focus especially on "suggest" and "evaluate" command words, which require you to apply knowledge to unfamiliar contexts rather than simply recall facts.
Several environmental and social factors influence how quickly and widely diseases spread within populations. Population density is a major factor — in densely populated areas, close contact between individuals facilitates the transmission of airborne pathogens like the influenza virus and Mycobacterium tuberculosis. Urbanisation has historically been associated with increased rates of respiratory infections and waterborne diseases. Climate also plays a crucial role, particularly for vector-borne diseases: warmer temperatures expand the geographical range of mosquito vectors, enabling malaria and dengue fever to spread into previously unaffected regions. Socioeconomic factors are equally important. Limited access to clean water and sanitation increases the risk of waterborne diseases such as cholera and typhoid. Malnutrition compromises immune function, making individuals more susceptible to infection and increasing disease severity. Overcrowded housing conditions facilitate the rapid spread of respiratory pathogens. Additionally, healthcare infrastructure — including vaccination coverage, availability of antibiotics, and disease surveillance systems — determines how effectively a society can prevent and control outbreaks. Global travel and trade have accelerated pathogen dispersal across continents, as demonstrated by the rapid spread of COVID-19, turning local outbreaks into global pandemics within weeks. When answering exam questions on disease control, always link specific interventions to the mode of transmission. For airborne diseases, mention improved ventilation and face masks. For waterborne diseases, discuss water treatment and sanitation infrastructure. For vector-borne diseases, describe insecticide-treated bed nets, draining stagnant water, and biological control using natural predators of mosquito larvae. This approach demonstrates the ability to apply knowledge rather than simply listing facts — a key skill for achieving top marks in OCR A-Level Biology.
