Edexcel GCSE
Health, disease and the development of medicines。
Edexcel GCSE Biology (1BI0) examines health and disease as Topic 5 of Paper 1, and the paper rewards precise comparisons rather than general descriptions; the specification was first taught from September 2016 and first assessed in 2018. A large share of lost marks comes from answers that describe a disease without naming the pathogen type or the transmission route. The ten sections below put the commonly confused pairs side by side, namely communicable against non-communicable disease, active against passive immunity, antibiotics against antivirals, and preclinical against clinical testing.
Communicable diseases are caused by pathogens and can pass from one organism to another, whereas non-communicable diseases such as coronary heart disease and type 2 diabetes are not transmitted between people; inherited conditions sit in the second group because no pathogen is involved. The quickest way to classify a disease in an exam is to ask two questions, namely whether a pathogen is present and whether a second person can catch it. HIV and tuberculosis remain the standard illustration that the two groups interact, since a weakened immune system makes the second infection far more likely.
Bacteria are single-celled organisms with a cell wall but no nucleus, viruses are far smaller and reproduce only inside a host cell, fungi have both a cell wall and a nucleus, and protists are single-celled eukaryotes; each group brings a standard example into the paper, namely cholera, influenza, athlete's foot and malaria. Malaria needs one extra step in the answer, because the Plasmodium protist is transmitted by mosquitoes, which act as the vector rather than the pathogen itself. A frequent error is the claim that antibiotics kill viruses, when antibiotics act only on bacteria.
Transmission route and control measure are taught as a pair, so water-borne cholera answers with clean water and sanitation while airborne influenza answers with ventilation; vector control for malaria means nets, insecticide and the removal of standing water. Answers score better when they follow the chain of infection and cover the source, the route and the susceptible host instead of listing hygiene in general. Vaccination works on the third link by making the body produce its own antibodies, which is why it is written separately from sanitation measures.
Defence works in two layers, since skin, mucous membranes and clotting act without recognising a specific pathogen while lymphocytes respond to one antigen at a time; phagocytes engulf pathogens whole, and lymphocytes that meet a matching antigen divide into antibody-producing cells and memory cells. Antibodies bind the antigen so that pathogens are marked, clumped or neutralised before phagocytes clear them. The wording matters in an exam, because antigen belongs to the pathogen whereas antibody is a protein the body makes itself.
Vaccination introduces antigens from weakened or inactivated pathogens, so the immune system makes its own antibodies and leaves memory cells behind; this is active immunity, slow to appear but long-lasting. Passive immunity works the other way, since ready-made antibodies are given directly or passed on in breast milk, which protects immediately but leaves no memory cells. Herd immunity follows from the active route, because a high enough vaccination rate leaves the pathogen too few susceptible hosts.
Antibiotics act only on bacteria, targeting structures such as the cell wall or bacterial protein synthesis, so they have no effect on viruses, which lack those targets; resistance is then natural selection in action, because a few bacteria already carry random mutations and survive while the sensitive ones are killed. The UK-commissioned Review on Antimicrobial Resistance, published in May 2016, estimated about 700,000 deaths a year from resistant infections and warned of 10 million a year by 2050. Answers should also add the practical steps, namely finishing a prescribed course, avoiding antibiotics for viral illness and cutting routine agricultural use.
Coronary heart disease results from fatty plaques narrowing the arteries, and the two interventions answer different questions, since statins lower blood cholesterol while stents hold a narrowed artery open; the treatments are not interchangeable in an answer. Cancer is usually tackled along three routes, namely surgery for solid tumours, radiotherapy that damages rapidly dividing cells and chemotherapy using drugs that interfere with cell division, and the last two damage healthy tissue as well. A common error is to treat a risk factor as a cause, because smoking and obesity raise the probability rather than guarantee the disease.
Lifestyle data show correlation rather than causation, which is a standard point in Paper 1, so a group that exercises more may also eat better and earn more, and the effect of exercise alone is easy to overstate; what can be stated with confidence is directional instead. Smoking raises the risk of lung cancer and coronary heart disease, long-term heavy drinking damages the liver, and poor diet with little exercise pushes up the rate of type 2 diabetes. BMI has limits as well, because it uses only height and mass and can mislabel a muscular person as overweight, which is why waist measurement or body fat is added in a full answer.
A new medicine is tested first on cultured cells and tissues and then on live animals, which establishes toxicity, effectiveness and a dose range before human trials begin; clinical trials then start with healthy volunteers at low doses to check safety, and move on to patients when the aim becomes effectiveness and the optimum dose. A placebo is an inactive treatment used as a comparison, and a double-blind trial hides the allocation from both patients and doctors so that expectation and observer bias do not distort the result. If both sides know which treatment is given, the trial is called open-label.
Willow bark contains salicin, which the body converts into salicylic acid and which later became aspirin, while foxglove supplies digitalis glycosides that treat heart failure but cause arrhythmia at excessive doses; the two examples show that a plant product can be both effective and dangerous. Traditional evidence is weak on its own, because the active substance, the dose and the natural recovery of the patient are all unknown, and isolation plus controlled trials are what establish effect and a safe dose. Purifying the active compound also allows standardised production and cuts side effects from other plant chemicals, which is why 'natural' does not equal 'safe'.
