Edexcel GCSE
What is Health?
In GCSE Edexcel Biology, the World Health Organization (WHO) defines health as "a state of complete physical, mental, and social well-being, and not merely the absence of disease or infirmity." This definition emphasises that health is not just about not being physically sick — it also includes mental well-being and positive social relationships.
Diseases can be divided into two major categories: communicable diseases and non-communicable diseases. Understanding the differences between these two types, their causes, and prevention methods is a core topic in the GCSE Biology exam.
Communicable Diseases
Communicable diseases are caused by pathogens and can spread from one person to another. In the Edexcel specification, you need to know four main types of pathogens: viruses, bacteria, fungi, and protists.
Viruses: Viruses are much smaller than bacteria. They invade host cells and replicate inside them, eventually destroying the cells. Typical viral diseases include measles, HIV, and tobacco mosaic virus (TMV). Measles spreads via droplets, with symptoms including fever and a red skin rash. HIV spreads through the exchange of body fluids, attacks the immune system, and leads to AIDS. TMV infects plants, causing leaf discolouration and impaired photosynthesis.
Bacteria: Bacteria are single-celled organisms that damage the host by releasing toxins. Important bacterial diseases include salmonella and gonorrhoea. Salmonella spreads through eating contaminated food and causes food poisoning symptoms. Gonorrhoea is a sexually transmitted disease with symptoms including pain when urinating and unusual discharge.
Fungi: Fungal pathogens can invade the skin or plant tissues. Rose black spot is a fungal disease that causes purple or black spots on rose leaves, which eventually turn yellow and fall off. Fungi spread through spores.
Protists: Protists are single-celled eukaryotic organisms. Malaria is caused by a protist called Plasmodium and is transmitted through the bite of female Anopheles mosquitoes. Symptoms include recurrent fever, chills, and headaches. Malaria remains a major global public health concern.
Non-Communicable Diseases
Non-communicable diseases cannot spread from person to person. They are typically caused by genetic factors, lifestyle choices, or environmental factors. The Edexcel specification focuses on cardiovascular disease, cancer, and lifestyle-related health issues.
Cardiovascular Disease: Coronary heart disease is one of the most common cardiovascular diseases. When coronary arteries narrow due to the buildup of fatty material (cholesterol), blood flow to the heart decreases, potentially causing angina or heart attacks. Stents and statins are two common treatments. A stent is a metal mesh tube inserted into the artery to keep it open. Statins are drugs that reduce cholesterol levels in the blood.
Heart valve disease can also occur — valves may become stiff and fail to open fully, or may develop leaks causing blood to flow backwards. In severe cases, they can be replaced with mechanical valves or biological valves (from pigs or cattle).
Cancer: Cancer is caused by uncontrolled cell division, leading to the formation of tumours. Benign tumours do not invade surrounding tissues and are usually not life-threatening. Malignant tumours are cancerous — they can invade neighbouring tissues and spread through the blood to other parts of the body (metastasis). Risk factors include smoking, excessive sun exposure (UV radiation), diet, and genetic predisposition.
Lifestyle Factors: Diet, exercise, smoking, and alcohol consumption significantly affect the risk of non-communicable diseases. A diet high in saturated fat increases cardiovascular disease risk. Lack of exercise is linked to obesity and type 2 diabetes. Smoking is a major risk factor for lung cancer, cardiovascular disease, and many other conditions. Excessive alcohol consumption can lead to liver cirrhosis and liver disease.
Transmission: Communicable diseases can spread from person to person via direct contact, airborne droplets, body fluids, contaminated food or water, or vectors (such as mosquitoes). Non-communicable diseases cannot be transmitted — they develop within an individual.
Causative Factors: Communicable diseases are caused by pathogens — viruses, bacteria, fungi, and protists. Non-communicable diseases result from a combination of factors: genetic predisposition, lifestyle choices (diet, exercise, smoking), environmental exposure, and the ageing process.
Treatment Approaches: Communicable diseases are typically treated with antibiotics (for bacteria) or antiviral drugs (for viruses), along with symptom relief. Vaccines can prevent many communicable diseases. Non-communicable disease treatment focuses more on long-term management: medication (such as statins, blood pressure drugs), surgical interventions (such as stents, bypass surgery), lifestyle modifications, and regular monitoring.
Prevention Strategies: Preventing communicable diseases centres on breaking the chain of transmission — good hygiene, clean water, vaccination, isolating infected individuals, and controlling vectors. Preventing non-communicable diseases primarily relies on a healthy lifestyle — balanced diet, regular exercise, not smoking, limiting alcohol intake, and regular health check-ups.
Human Defence Systems
The human body has multiple layers of defence against pathogens. The first line of defence consists of non-specific defences, including the skin (physical barrier), mucus and cilia in the nose and trachea (trap and remove pathogens), stomach acid (kills ingested pathogens), and lysozyme enzymes in tears.
If pathogens breach the first line of defence, the immune system activates specific defences. White blood cells work in three ways: phagocytosis — engulfing and digesting pathogens; producing antibodies — locking onto and marking specific pathogens for destruction; and producing antitoxins — neutralising toxins released by pathogens.
Vaccination and Antibiotics
Vaccines contain dead or weakened pathogens. After vaccination, the immune system produces antibodies against that pathogen. If the real pathogen is encountered later, memory cells can rapidly produce large quantities of antibodies, preventing disease. Herd immunity occurs when enough of the population is vaccinated that the disease struggles to spread, protecting unvaccinated individuals.
Antibiotics are used to treat bacterial infections. They work by killing bacteria or preventing them from reproducing. Key exam point: antibiotics do NOT work against viral diseases! This is because viruses hide inside host cells, and antibiotics cannot target them without damaging the host cells. Antibiotic resistance is a growing problem — bacteria evolve resistance through natural selection, leading to the emergence of "superbugs."
