Communicable Diseases 传染病
Diseases caused by pathogens that can be transmitted from one organism to another. Examples include measles, HIV, salmonella, and malaria.
Non-Communicable Diseases
Diseases not caused by pathogens and not transmitted between individuals. Examples include cancer, heart disease, and diabetes. Risk factors like smoking, diet, and genetics play a role.
**Pathogens: The Four Types 四大病原体** Pathogens are microorganisms that cause infectious diseases. The AQA specification requires you to know four categories: bacteria, viruses, fungi, and protists. Each type invades the host and causes damage in distinct ways. Bacteria are single-celled organisms that reproduce rapidly inside the body. They can produce toxins that damage tissues and make us feel ill. Salmonella causes food poisoning by producing toxins in the gut. Gonorrhoea is a sexually transmitted bacterial disease. Viruses are much smaller than bacteria and cannot reproduce on their own. They invade host cells, hijack the cellular machinery, and burst the cells when they replicate. Measles is a highly contagious viral disease spread by droplet infection; symptoms include fever and a red skin rash. HIV attacks the immune system itself, destroying white blood cells and leading to AIDS. Tobacco Mosaic Virus (TMV) affects plants, causing mosaic patterns on leaves that reduce photosynthesis. Fungi can be single-celled or multicellular. Rose black spot is a fungal disease that causes purple or black spots to develop on leaves of rose plants, reducing photosynthesis. It spreads via water or wind. Protists are eukaryotic single-celled organisms. Malaria is caused by a protist pathogen called Plasmodium, which is transmitted by female Anopheles mosquitoes. The parasite invades red blood cells, causing recurrent fevers. Control strategies include insecticide-treated bed nets and antimalarial drugs.
When asked about disease prevention, always link back to the mode of transmission. For airborne diseases, mention isolation and hygiene. For vector-borne diseases, mention vector control. For waterborne diseases, mention sanitation and clean water.
**Human Defence Systems 人体防御系统** The human body has evolved a multi-layered defence system against pathogens. The first line of defence consists of physical and chemical barriers. The skin acts as a physical barrier, and if cut, platelets form clots to seal the wound. The nose traps particles with mucus and hairs. The trachea and bronchi are lined with ciliated cells that sweep mucus up to the throat where it is swallowed. The stomach produces hydrochloric acid that kills most pathogens in food and drink. The second line of defence is the immune system, which involves white blood cells. Three key mechanisms are tested in AQA exams: phagocytosis, antibody production, and antitoxin production. Phagocytes (a type of white blood cell) engulf and digest pathogens — a process called phagocytosis. Lymphocytes produce antibodies that are specific to antigens on the pathogen surface. Each antibody has a complementary shape to a specific antigen, locking onto it like a key in a lock. Once bound, antibodies cause pathogens to clump together for phagocytes to destroy. Lymphocytes also produce antitoxins that neutralise toxins released by bacteria.
Antibodies do not kill pathogens directly. They bind to antigens and cause pathogens to clump together (agglutination), making it easier for phagocytes to engulf them. Always describe this sequence in full on 4-6 mark questions.
**Vaccination 疫苗接种** Vaccination works by introducing a weakened or inactive form of a pathogen into the body. This stimulates the lymphocytes to produce antibodies specific to the pathogen's antigens. Memory lymphocytes remain in the blood long after the initial exposure. If the same pathogen invades again, memory cells recognise the antigen and mount a rapid secondary immune response, producing large quantities of antibodies before symptoms develop. Herd immunity occurs when a large proportion of the population is vaccinated, making it difficult for the pathogen to spread because there are too few susceptible hosts. This protects vulnerable individuals who cannot be vaccinated, such as newborns or those with compromised immune systems. **Antibiotics and Painkillers 抗生素与止痛药** Antibiotics are drugs that kill bacteria or inhibit their growth. Penicillin, discovered by Alexander Fleming in 1928, was the first antibiotic. Antibiotics are effective only against bacteria — they do not work on viruses because viruses reproduce inside host cells and lack the cellular structures that antibiotics target. This is an extremely common exam question — do not confuse antibiotics with antiviral drugs. Painkillers (such as aspirin and paracetamol) treat the symptoms of disease but do not kill pathogens. They are used to relieve pain and reduce fever, allowing the immune system to do the actual work of fighting the infection. **Antibiotic Resistance 抗生素耐药性** Antibiotic resistance is one of the most pressing global health challenges and a guaranteed exam topic. Bacteria can develop resistance through random mutations. When antibiotics are used, non-resistant strains are killed, leaving resistant strains to survive and reproduce. This is a classic example of natural selection. Resistant bacteria then pass on the resistance genes to their offspring through binary fission. Over time, populations of antibiotic-resistant bacteria increase, making infections harder to treat. MRSA is a well-known antibiotic-resistant bacterium commonly found in hospitals. MRSA(耐甲氧西林金黄色葡萄球菌)是医院中常见的耐药菌。 To slow the development of resistance, doctors prescribe antibiotics only for serious bacterial infections and patients must complete the full course even if they feel better. The development of new antibiotics is also critical, though the rate of discovery has slowed dramatically. **Discovery and Development of Drugs 新药研发** New drugs must undergo rigorous testing before they can be prescribed. Preclinical testing involves experiments on cells, tissues, and live animals to assess toxicity, efficacy, and dosage. Clinical trials then test the drug on healthy volunteers and patients. A double-blind trial is the gold standard: neither the patient nor the doctor knows who receives the active drug and who receives a placebo, eliminating bias. Placebos are used as a control to confirm that any observed effect is due to the drug itself, not the patient's expectations. **Plant Diseases and Defences 植物病害与防御** Plants also face threats from pathogens. Common signs of plant disease include stunted growth, spots on leaves, areas of decay, abnormal growths (galls), and discolouration. Plants can be infected by viruses (TMV), bacteria (Agrobacterium), fungi (rose black spot, powdery mildew), and insects (aphids as vectors). Nutrient deficiencies can also cause disease-like symptoms: nitrate deficiency causes stunted growth, while magnesium deficiency causes chlorosis due to insufficient chlorophyll. Plants have evolved physical, chemical, and mechanical defences. Physical barriers include the waxy cuticle and bark. Chemical defences include antibacterial compounds and poisons. Mechanical defences include thorns, hairs, and leaf drooping (mimosa plants). These are passive defences — plants cannot move away from danger, so they rely on these built-in protective mechanisms.

An organism that transmits a pathogen from one host to another (e.g. mosquito) Mastering the Infection and Response topic requires both factual recall and the ability to apply concepts to unfamiliar scenarios. The AQA exam often presents data on disease outbreaks, vaccination rates, or antibiotic resistance trends and asks you to analyse and evaluate. Practice interpreting graphs and calculating percentage changes — these maths skills carry marks even in the biology paper. Remember: all communicable diseases follow the same fundamental pattern of transmission, infection, symptoms, and control. Once you internalise this pattern, you can reason through any pathogen the exam throws at you. 🎓 需要IGCSE/A-Level/IB全科辅导?
