GCSE AQA Biology
Communicable diseases are a core topic in GCSE AQA Biology, covering how pathogens spread, how the body defends itself, and how we prevent and treat infections. This article breaks down every key concept in the Infection and Response unit: from the four pathogen types to vaccination, antibiotics, and drug development.

A pathogen is a microorganism that causes infectious disease. Pathogens can be bacteria, viruses, fungi, or protists. They spread through direct contact, airborne droplets, contaminated food and water, or via vectors like mosquitoes. Bacteria reproduce rapidly inside the body and produce toxins that damage cells and tissues. Viruses, much smaller than bacteria, invade host cells and hijack their machinery to replicate, eventually bursting the cell and releasing thousands of new virus particles. Fungi can grow on or in the body and produce spores. Protists are eukaryotic parasites that often require a vector for transmission. ### Viral Diseases 病毒性疾病 **Measles 麻疹**: A highly contagious viral disease spread by airborne droplets. Symptoms include fever and a red skin rash. Measles can be fatal if complications like pneumonia or encephalitis arise. Most children are vaccinated with the MMR vaccine, which has dramatically reduced measles incidence worldwide. **HIV/AIDS**: Human Immunodeficiency Virus attacks the immune system's helper T cells, weakening the body's ability to fight off other infections. HIV is transmitted through bodily fluids during sexual contact, needle sharing, or from mother to child. Antiretroviral drugs control viral replication, allowing patients to live long healthy lives, but there is currently no cure. Late-stage HIV infection, when the immune system is severely compromised, is called AIDS. **Tobacco Mosaic Virus (TMV) 烟草花叶病毒**: A plant pathogen that infects chloroplasts, causing a distinctive mosaic pattern of discolouration on leaves. TMV reduces photosynthesis and crop yield. Unlike animal viruses, TMV spreads through direct contact between plants or via contaminated tools. ### Bacterial Diseases 细菌性疾病 **Salmonella Food Poisoning 沙门氏菌食物中毒**: Caused by Salmonella bacteria ingested through undercooked poultry, eggs, or contaminated food. Symptoms include fever, abdominal cramps, vomiting, and diarrhoea. Prevention relies on proper food hygiene: cooking food thoroughly, avoiding cross-contamination, and refrigerating perishable items promptly. In the UK, most chickens are vaccinated against Salmonella. **Gonorrhoea 淋病**: A sexually transmitted bacterial disease caused by Neisseria gonorrhoeae. Symptoms include thick yellow or green discharge from the genitals and pain during urination. Untreated gonorrhoea can lead to infertility. Previously treated easily with penicillin, many strains are now antibiotic-resistant. Prevention includes barrier contraception and prompt antibiotic treatment. ### Fungal and Protist Diseases 真菌与原生物疾病 **Rose Black Spot 玫瑰黑斑病**: A fungal disease affecting rose plants, causing purple or black spots to develop on leaves, which then turn yellow and drop off. The fungus spreads via spores carried by wind or water splash. Gardeners treat it with fungicides and by removing and burning affected leaves to prevent spore dispersal. **Malaria 疟疾**: Caused by Plasmodium protists, transmitted to humans by female Anopheles mosquitoes acting as vectors. The protist reproduces inside liver cells and red blood cells, causing recurrent fevers, shaking chills, and anaemia. Malaria kills hundreds of thousands of people annually, predominantly children in sub-Saharan Africa. Control strategies include insecticide-treated mosquito nets, indoor residual spraying, and antimalarial drugs. The life cycle involves both human and mosquito hosts: the protist undergoes sexual reproduction in the mosquito gut and asexual reproduction in human cells. The body has multiple lines of defence against pathogens. The first line includes physical and chemical barriers: skin acts as a waterproof physical barrier, nasal hairs and mucus trap particles, cilia in the trachea sweep mucus upward, and stomach acid kills ingested pathogens.

If pathogens breach these barriers, the immune system deploys white blood cells. There are three main mechanisms: (1) Phagocytosis: phagocytes engulf and digest pathogens non-specifically. (2) Antibody production: B lymphocytes produce specific antibodies that bind to antigens on the pathogen surface, marking them for destruction. (3) Antitoxin production: white blood cells neutralise toxins released by bacteria. Vaccination introduces a dead or weakened form of a pathogen into the body. This stimulates the immune system to produce antibodies and memory lymphocytes without causing illness. If the same pathogen later infects th e body, memory cells recognise it rapidly and produce large quantities of specific antibodies before symptoms develop, providing immunity. Herd immunity occurs when a large proportion of the population is vaccinated, reducing pathogen spread and protecting those who cannot be vaccinated, such as newborns or immunocompromised individuals. Vaccines have eradicated smallpox and drastically reduced diseases like polio, measles, and tetanus. Antibiotics are drugs that kill bacteria or prevent their growth without harming human cells. They target features unique to bacterial cells, such as cell wall synthesis. Penicillin, discovered by Alexander Fleming in 1928, was the first widely used antibiotic. Antibiotics are ineffective against viruses because viruses lack the cellular structures antibiotics target. The emergence of antibiotic-resistant bacteria is a major global health concern. Resistance arises through natural selection: when a population of bacteria is exposed to an antibiotic, resistant mutants survive and reproduce, passing resistance genes to offspring. MRSA (Methicillin-resistant Staphylococcus aureus) is a well-known example. To slow resistance, patients must complete prescribed courses even if they feel better, and doctors should avoid prescribing antibiotics for viral infections. Painkillers such as paracetamol relieve symptoms but do not kill pathogens. New drugs undergo rigorous testing before they are licensed. Preclinical testing uses cells, tissues, and live animals to assess toxicity, efficacy, and dosage. Clinical trials then test the drug on healthy volunteers and patients in phases. Phase 1 (small group of healthy volunteers): safety and dosage. Phase 2 (larger group of patients): efficacy and side effects. Phase 3 (large-scale patients): comparison against existing treatments. Double-blind trials are used to eliminate bias: neither the patients nor the doctors know who receives the active drug and who receives a placebo. This ensures results are objective. Placebos are inert substances identical in appearance to the drug, controlling for the psychological expectation of improvement. Preclinical

They are used in pregnancy tests (detecting hCG in urine), cancer treatment (targeting cancer cell antigens with attached drugs or radioactive substances), and disease diagnosis (detecting specific pathogen antigens in blood samples). Exam questions on Infection and Response frequently ask students to explain how diseases spread and how to prevent them. A classic 4-6 mark question: "Describe how the body defends itself against pathogens." Your answer must cover physical barriers (skin, mucus, cilia, stomach acid) AND the immune system (phagocytosis, antibodies, antitoxins) for full marks. Another common pitfall: confusing antibiotics with antibodies. Antibiotics kill bacteria; antibodies are proteins made by white blood cells that bind to antigens. Students often write "antibiotics" when they mean "antibodies" and vice versa. Know the difference: antibiotics are medicines you take; antibodies are produced by your own body. When describing vaccination, always include the memory cell step. A full answer: (1) dead/weakened pathogen injected, (2) white blood cells produce specific antibodies, (3) memory cells remain in the blood, (4) on re-exposure, memory cells rapidly produce large amounts of antibodies, preventing illness. For drug development questions, remember the sequence: preclinical → Phase 1 → Phase 2 → Phase 3, and explain why each step exists. Also know that double-blind trials and placebos are used to eliminate bias and the placebo effect. Check off each item as you revise 复习时逐项打勾: - [ ] Can I name all four pathogen types and give a disease example for each? - [ ] Can I explain how measles, HIV, and TMV spread and their symptoms? - [ ] Can I describe the life cycle of malaria including the vector? - [ ] Can I distinguish between antibiotics, painkillers, and antibodies? - [ ] Can I outline the three lines of human defence? - [ ] Can I explain how vaccination works, including herd immunity? - [ ] Can I describe the stages of drug testing and why double-blind trials are used? - [ ] Can I explain how antibiotic resistance develops via natural selection?
