### 1. Pathogens: The Four Main Types
Pathogens are microorganisms that cause infectious disease. The OCR specification requires you to know four types, each with distinct characteristics and representative organisms.
**Bacteria 细菌**: Prokaryotic cells that lack membrane-bound organelles. They reproduce rapidly by binary fission and damage host tissues by releasing toxins. *Mycobacterium tuberculosis* causes tuberculosis (TB), damaging lung tissue and triggering an immune response that destroys alveoli. *Vibrio cholerae* produces a toxin that opens chloride ion channels in intestinal cells, causing water to flood into the gut lumen by osmosis, leading to severe diarrhoea.
**Viruses 病毒**: Non-living particles consisting of genetic material (DNA or RNA) surrounded by a protein capsid. They invade host cells, hijack the cellular machinery to replicate, then burst out, destroying the host cell. HIV targets helper T cells, crippling the immune system. Tobacco Mosaic Virus (TMV) infects plants, causing mosaic-like discolouration and stunted growth by damaging chloroplasts.
**Fungi 真菌**: Eukaryotic organisms that often cause disease by producing enzymes that digest living tissue. They can be unicellular (yeasts) or multicellular (moulds). Fungal diseases in plants include potato blight (*Phytophthora infestans*), which destroyed Ireland's potato crop in the 1840s, and black sigatoka (*Mycosphaerella fijiensis*), which reduces banana yields by up to 50%. Both spread via airborne spores.
*Plasmodium* species cause malaria by invading red blood cells, liver cells, and using female *Anopheles* mosquitoes as vectors. The parasite has a complex life cycle alternating between human and mosquito hosts, making it remarkably difficult to eradicate.
### 2. Transmission Routes
Understanding how pathogens spread is crucial for designing effective control strategies. The OCR exam expects you to connect transmission mechanisms to prevention methods.
**Direct transmission**: Physical contact (HIV via sexual contact, bacterial meningitis via kissing), droplet infection (TB and influenza via coughs and sneezes), faecal-oral route (cholera via contaminated water), and transplacental (HIV from mother to foetus).
**Indirect transmission**: Through fomites (contaminated surfaces and objects like bedding or shared towels), vectors (mosquitoes for malaria, ticks for Lyme disease), and contaminated food or water (salmonella, cholera).
**Plant-specific transmission**: Soil contamination (fungal spores surviving in soil), wind dispersal (spores of black sigatoka and potato blight travelling miles), water splash, insect vectors, and contaminated tools or hands during farming.
### 3. Plant Defences Against Pathogens
Plants lack circulatory and immune systems but possess an impressive array of passive and active defences.
**Passive physical defences**: Waxy cuticle preventing water accumulation (which would support pathogen growth), thick bark, cellulose cell walls acting as physical barriers, stomatal closure to prevent pathogen entry, and thorns deterring insect vectors.
**Passive chemical defences**: Antimicrobial compounds (tannins in bark, phenolic compounds), enzyme inhibitors, and alkaloids that are toxic to herbivores and pathogens.
**Active defences**: When pathogens breach passive defences, plants mount stronger responses. Callose deposition strengthens cell walls at the infection site. Hypersensitive response involves programmed cell death of infected cells, isolating the pathogen. Oxidative bursts produce hydrogen peroxide to kill pathogens directly and signal neighbouring cells.
### 4. The Human Immune System: Non-Specific Defences
The body's first lines of defence are non-specific — they respond to all pathogens generically without distinguishing between different types.
**Primary barriers**: Skin (keratinised, dry, slightly acidic pH creating hostile environment), mucous membranes (trapping pathogens in sticky mucus, cilia sweep them away), lysozyme enzymes in tears and saliva (breaking down bacterial cell walls), stomach acid (denaturing pathogens' proteins).
**Inflammatory response**: When tissues are damaged, mast cells release histamine, causing vasodilation and increased capillary permeability. This results in redness, swelling, heat, and pain. Increased blood flow brings more phagocytes and plasma proteins to the site. Cytokines attract phagocytes by chemotaxis.
**Phagocytosis**: Neutrophils and macrophages engulf pathogens. The phagocyte's membrane extends around the pathogen, forming a phagosome. Lysosomes fuse with the phagosome, releasing lytic enzymes (lysozyme) and hydrogen peroxide that destroy the pathogen. The harmless breakdown products are absorbed or exocytosed.

### 5. The Specific Immune Response
This is the most challenging section of the Disease topic. You must distinguish between the humoral (antibody-mediated) and cell-mediated responses, and explain how they work together.
**Antigens and self/non-self**: Antigens are molecules (usually proteins or polysaccharides) on pathogen surfaces that the immune system recognises as foreign. The body learns to recognise its own cells through the deletion of self-reactive lymphocytes during development — failure of this process leads to autoimmune disease.
**Cell-mediated response (T lymphocytes)**: Phagocytes that have engulfed pathogens present antigens on their surface via MHC Class II molecules, becoming antigen-presenting cells (APCs). Helper T cells with complementary CD4 receptors bind to these antigens and are activated. Activated helper T cells release cytokines that: stimulate B cells to divide and differentiate, activate cytotoxic T cells (which kill infected cells by releasing perforin), and enhance phagocytosis by macrophages.
**Humoral response (B lymphocytes)**: B cells have antibodies on their surface that bind to complementary antigens. When a B cell binds its specific antigen and is stimulated by a helper T cell, it undergoes clonal selection and clonal expansion, dividing rapidly into plasma cells and memory cells. Plasma cells secrete large quantities of antibodies into the blood and lymph. Memory B cells persist for decades, enabling rapid secondary response upon re-exposure.
**Primary vs Secondary Response**: The primary response is slow (lag phase of several days) and produces lower antibody concentrations, mainly IgM then IgG. The secondary response is rapid, produces much higher antibody concentrations (mainly IgG), and symptoms may be avoided entirely. This is the principle behind vaccination.
**Antibody structure and function**: Antibodies are Y-shaped proteins made of 4 polypeptide chains (2 heavy, 2 light) held together by disulfide bridges. The variable region at the tips is the antigen-binding site with a complementary shape to the specific antigen. The constant region is recognised by phagocytes. Antibodies work by: agglutination (clumping pathogens together for easier phagocytosis), neutralisation (blocking pathogen binding sites), opsonisation (marking pathogens for destruction), and activation of the complement system.
### 6. Vaccination and Herd Immunity
Vaccination involves introducing antigens (whole inactivated pathogens, weakened strains, isolated antigens, or mRNA coding for antigens) to stimulate a primary immune response and generate memory cells without causing the disease. This is active immunity — the individual's own immune system produces the antibodies.
**Herd immunity**: When a sufficiently high proportion of a population is vaccinated (typically 80-95% depending on the disease's R0 value), transmission chains break, protecting even unvaccinated individuals. This is essential for protecting immunocompromised patients who cannot receive live vaccines.
**Ethical considerations**: The OCR exam may ask you to discuss the ethics of mandatory vaccination, the balance between individual autonomy and public health, and the consequences of declining vaccination rates (as seen with measles resurgence). Always frame your answer considering both sides.
### 7. Antibiotics and Resistance
Antibiotics are chemicals that kill or inhibit bacterial growth without harming human cells. They exploit differences between prokaryotic and eukaryotic cells: penicillin inhibits cell wall synthesis (peptidoglycan cross-linking), tetracyclines inhibit protein synthesis by binding to 70S ribosomes, and quinolones inhibit DNA replication.
**Important**: Antibiotics are ineffective against viruses because viruses have no metabolic pathways of their own — they use the host cell's machinery, which antibiotics cannot target without harming human cells. The OCR examiners love to test this distinction.
**Antibiotic resistance**: Bacteria evolve resistance through natural selection. Resistant mutations occur randomly. When antibiotics are used, susceptible bacteria die while resistant ones survive and reproduce, passing on resistance genes. Horizontal gene transfer via conjugation (sharing plasmids) accelerates spread. MRSA (Methicillin-resistant *Staphylococcus aureus*) is a prominent example. Strategies to reduce resistance include completing full antibiotic courses, reducing unnecessary prescriptions, and restricting agricultural use.
MRSA(耐甲氧西林金黄色葡萄球菌)是突出例子。
