📚 The Structure of the Kidney |
The kidneys are paired excretory organs that play a central role in homeostasis. They remove nitrogenous waste such as urea, regulate water and electrolyte balance, maintain acid-base equilibrium, and secrete hormones including erythropoietin. A clear understanding of kidney structure is essential in A-Level Biology because the arrangement of blood vessels and tubules directly explains how filtration, selective reabsorption and osmoregulation occur.
Examiners rarely ask you to simply label a diagram. They ask you to link a structural feature, such as the width of the afferent arteriole or the length of the loop of Henle, to a functional consequence. This article builds the kidney from the outside inwards, so that every structure you learn is immediately tied to a process you must explain.
1. Position, Shape and the Hilum |
Each kidney is a bean-shaped organ about 10-12 cm long, 5-7 cm wide and 3 cm thick, weighing roughly 150 g in an adult. The two kidneys lie retroperitoneally on the posterior abdominal wall, one on each side of the vertebral column, between the levels of T12 and L3. The right kidney usually lies slightly lower than the left because the liver occupies space above it.
The upper poles of the kidneys are protected by the eleventh and twelfth ribs, and the organs are embedded in a thick cushion of perirenal fat. This fat is not merely packing material: it holds the kidneys in position and absorbs mechanical shock, which is why very thin patients may develop a mobile or 'floating' kidney.
The concave medial border contains a slit called the hilum, through which the renal artery enters, and the renal vein, ureter, lymph vessels and nerves leave. The hilum opens into a cavity called the renal sinus, which is packed with fat and contains the renal pelvis. Because the renal artery, renal vein and ureter all converge at the hilum, this point is the classic landmark used to identify the medial side of a kidney in an unfamiliar diagram.
The kidneys receive about 20-25% of the resting cardiac output, despite making up less than 1% of body mass. This disproportionate blood flow is the first structural clue that the kidney is built around a filtration process that requires a high and constant pressure.
2. The Three Coverings and the Route to the Bladder |
From the inside outwards, each kidney is wrapped by three concentric coverings. The renal capsule is a tough, fibrous, transparent sheath that adheres tightly to the kidney surface and limits swelling. Outside it lies the adipose capsule, a fatty layer that cushions the organ, and outermost is the renal fascia, a dense connective tissue sheet that anchors the kidney to the abdominal wall.
Urine formed in the kidney does not simply drip out. It drains from the collecting ducts into the renal pelvis, then passes down the ureter, a muscular tube lined with transitional epithelium. Peristaltic contractions of the smooth muscle in the ureter wall push urine to the urinary bladder, and from there it leaves the body through the urethra. No active pumping by the kidney itself is required.
3. Internal Gross Anatomy: Cortex, Medulla and Pelvis |
A longitudinal section reveals two distinct zones. The outer renal cortex is a pale, granular layer about 1 cm thick that also extends inwards between the pyramids as the renal columns of Bertin. The inner renal medulla consists of 8-18 cone-shaped renal pyramids, whose striated appearance reflects the packed parallel loops of Henle and collecting ducts running through them.
The base of each pyramid faces the cortex and the apex, called the renal papilla, points towards the renal sinus. Each papilla is cupped by a minor calyx; several minor calyces unite to form a major calyx, and the major calyces merge into the funnel-shaped renal pelvis, which narrows to become the ureter.
The difference in appearance between cortex and medulla is a functional fingerprint. The granular cortex contains the renal corpuscles, where filtration occurs and where the blood is under highest pressure, while the striated medulla contains the structures responsible for concentrating the urine by building an osmotic gradient.
| Position 位置 | Outer layer plus renal columns 外层及肾柱 | Inner pyramids and papillae 内侧锥体与乳头 |

4. The Nephron: The Functional Unit of the Kidney |
Each human kidney contains roughly one million nephrons, and each nephron is a blind-ended tubule about 3-5 cm long whose wall is a single layer of epithelial cells resting on a basement membrane. The nephron is the functional unit of the kidney because one nephron alone can carry out the complete sequence of filtration, selective reabsorption, secretion and concentration.
Structurally, a nephron is divided into the renal corpuscle (the glomerulus enclosed by Bowman's capsule), the proximal convoluted tubule, the loop of Henle, and the distal convoluted tubule. The collecting duct is not strictly part of any single nephron: several distal convoluted tubules from different nephrons drain into one collecting duct, which then runs down through the medulla to a papilla.
Two types of nephron are recognised. Cortical nephrons make up about 85% of the total, have short loops of Henle that barely enter the medulla, and are mainly responsible for reabsorbing useful solutes. Juxtamedullary nephrons, about 15%, have very long loops that penetrate deep into the medulla and are chiefly responsible for producing concentrated urine.
- Loop of Henle 髓袢 - creation of a medullary osmotic gradient 建立髓质渗透梯度
5. The Renal Corpuscle: Glomerulus and Bowman's Capsule |
The renal corpuscle is a spherical structure roughly 0.2 mm across, and it is the only part of the nephron specialised for filtration rather than transport. It consists of a knot of about 50 fenestrated capillaries, the glomerulus, invaginated into a double-walled cup, Bowman's capsule.
The blood supplying the glomerulus arrives through the afferent arteriole, which has a wider lumen than the efferent arteriole draining it. This asymmetry is the key to ultrafiltration: blood enters under systemic arterial pressure but leaves against a narrower exit, so the hydrostatic pressure inside the glomerular capillaries remains high, roughly 8 kPa above the pressure in the capsular space.
Bowman's capsule has an outer parietal layer of simple squamous epithelium and an inner visceral layer made of specialised cells called podocytes. Between the two layers lies the capsular (Bowman's) space, which collects the filtrate and drains it directly into the proximal convoluted tubule. Podocytes have long primary processes that branch into foot processes, and the gaps between adjacent foot processes are called filtration slits.
The whole capsule is therefore a shaped collecting chamber rather than a simple balloon. Its cup form increases the surface area available for filtration, and the narrow lumen of the proximal tubule that follows ensures the filtrate is moved along without stasis.
6. The Filtration Barrier and How Structure Explains Ultrafiltration |
Three layers together form the filtration barrier between the blood plasma and the capsular space. The first is the fenestrated endothelium of the glomerular capillaries, whose pores are large enough to stop blood cells but not plasma proteins. The second is the glomerular basement membrane, a meshwork of collagen and glycoproteins that acts as the main molecular sieve. The third is the podocyte layer with its filtration slits.
The barrier is both size-selective and charge-selective. Molecules smaller than about 5 nm and with a molecular mass below roughly 68 kDa, such as water, glucose, amino acids, urea and ions, pass freely. Larger proteins such as albumin and globulins, and all blood cells, are normally retained in the plasma; the negative charge on the basement membrane further repels negatively charged plasma proteins.
Because the filtrate is essentially plasma minus large proteins and cells, it is described as an ultrafiltrate. The process is passive, driven purely by hydrostatic pressure, and requires no ATP. That is a favourite exam point: ultrafiltration is pressure-driven and energy-independent, whereas every later stage of reabsorption is active and ATP-dependent.
Filtration rate is remarkable: the glomerular filtration rate in a healthy adult is about 125 cm³ min⁻¹, which means the kidneys filter roughly 180 dm³ of fluid every day. Since daily urine output is only about 1.5 dm³, more than 99% of the filtrate must be reabsorbed. Structure and function are inseparable here - such a high filtration rate depends absolutely on the wide afferent and narrow efferent arterioles.
7. The Proximal Convoluted Tubule and Selective Reabsorption |
The proximal convoluted tubule is the longest and most coiled section of the nephron, and it is the site of bulk selective reabsorption. Its wall is a single layer of cuboidal epithelial cells, each packed with mitochondria and bearing a dense brush border of microvilli on the luminal surface. The basal membrane is folded into deep
