X-ray imaging remains one of the most widely used diagnostic tools in medicine. The quality of an X-ray image directly affects a clinician's ability to detect pathology. Factors such as contrast, sharpness, noise, and patient dose must be carefully balanced. This article explores the physical principles and practical techniques used to improve X-ray image quality, following the A-Level CIE Physics syllabus.
Image quality in radiography is defined by three main attributes: contrast, spatial resolution (sharpness), and noise. Contrast is the difference in optical density between adjacent regions, allowing differentiation of tissues. Resolution refers to the ability to distinguish small details, while noise manifests as random variations in brightness that obscure information. Improving an X-ray image requires optimising all three without exposing the patient to unnecessary radiation.
2. Kilovoltage Peak (kVp) and Contrast |
The kilovoltage peak applied across the X-ray tube determines the maximum photon energy and the shape of the X-ray spectrum. A lower kVp produces lower-energy photons that are more readily absorbed by tissues via the photoelectric effect. This enhances contrast because different tissues (bone, soft tissue, fat) have significantly different attenuation coefficients at low energies. However, too low a kVp increases patient skin dose and can produce a noisy image if insufficient photons reach the detector. Raising kVp increases beam penetration and reduces contrast, as Compton scattering becomes dominant, producing more forward-scattered radiation. To improve contrast, the lowest kVp that still provides adequate exposure should be selected, often around 60–80 kVp for extremities and 100–120 kVp for thicker body parts.
3. Tube Current and Exposure Time (mAs) |
Tube current, measured in milliamperes (mA), multiplied by the exposure time (seconds) gives the mAs value, which is directly proportional to the total number of X-ray photons produced. Increasing mAs improves image quality by reducing quantum mottle (noise), because more photons reaching the detector yield a smoother image. However, this also increases patient dose linearly. In practice, a compromise is reached: an mAs high enough to keep noise at an acceptable level but as low as reasonably achievable (ALARA principle). Modern automatic exposure control (AEC) systems terminate the exposure once a preset detector dose is reached, optimising the mAs for each patient’s thickness.
4. Scattered Radiation and Anti-scatter Grids |
When an X-ray beam passes through the patient, a significant portion of photons undergoes Compton scattering, producing radiation that deviates from the original path. This scattered radiation reaches the detector from random directions, reducing contrast and creating a fog-like background. An anti-scatter grid is placed between the patient and the detector to absorb scattered photons while allowing primary photons to pass. A typical grid consists of alternating strips of lead and radiolucent material, angled to match the beam divergence. Using a grid improves contrast noticeably but requires an increase in mAs by a factor of 2–4 because some primary photons are also attenuated. For thinner body parts or paediatric imaging, grids are sometimes omitted to avoid unnecessary dose.
5. Collimation and Field Size |
Collimation restricts the X-ray beam to the area of clinical interest. Narrowing the field size reduces the volume of tissue irradiated, thereby decreasing the amount of scattered radiation produced. Less scatter means higher contrast and lower dose to the patient. Collimators are typically adjustable lead shutters mounted near the X-ray tube window. The light field projected by the collimator illuminates the area to be exposed, allowing the radiographer to accurately set the beam boundaries. Proper collimation is one of the simplest yet most effective methods for improving image quality and radiation safety.
6. Using Intensifying Screens |
In film-screen radiography, intensifying screens convert X-ray photons into visible light to expose the film, greatly reducing the mAs required. A typical intensifying screen is a layer of phosphor material, such as gadolinium oxysulfide or calcium tungstate, that fluoresces under X-rays. High-speed screens emit more light per X-ray photon, requiring less radiation but producing a less sharp image because light spreads laterally in the thicker phosphor layer. To improve sharpness, fine-detail screens with thinner phosphor layers and smaller crystal grains are used for extremities. The compromise is a slower speed, demanding a higher dose. Modern digital detectors use scintillator layers coupled to photodiodes, applying the same principle but with better sensitivity and lower noise.
7. Focal Spot Size and Geometric Unsharpness |
The X-ray tube’s focal spot is the area of the anode struck by electrons, from which X-rays are emitted. A smaller focal spot produces a narrower penumbra (edge unsharpness) on the image, improving spatial resolution. Geometric unsharpness U₉ can be expressed as:
U₉ = f × (b / a)
where f is the focal spot size, a is the focus-to-object distance, and b is the object-to-detector distance. To minimise U₉, use a small focal spot, position the anatomy close to the detector, and maximise the distance from the source. However, small focal spots limit the tube current (to avoid overheating), so they are reserved for high-detail views where higher mAs is acceptable.
U₉ = f × (b / a)
8. Air Gap Technique |
Instead of a physical grid, an air gap can be used to reduce the effects of scattered radiation. If the detector is moved a distance of 15–30 cm away from the patient, scattered photons travelling obliquely are more likely to miss the detector, while primary photons maintain a straighter path. This technique improves contrast without the grid-induced dose penalty. However, it introduces magnification and increased geometric unsharpness, so it must be used with a larger source-to-object distance and a small focal spot to maintain sharpness. The air gap is particularly useful in chest radiography and some paediatric exams.
9. Image Processing: Windowing |
In digital radiography, the acquired data consists of a wide range of pixel values representing varying attenuation. Windowing is an image processing technique that maps a selected range of pixel values to the visible grey scale. The window level (centre) sets the midpoint of displayed attenuation, while the window width determines the range of values shown. A narrow window width enhances contrast because small attenuation differences are spread over more grey levels. However, if the width is too narrow, image noise becomes more apparent. Adjusting window settings allows clinicians to optimise the visibility of specific tissues without re-exposing the patient.
10. Digital Detectors and Noise Reduction |
Modern flat-panel detectors based on amorphous silicon (a-Si) or amorphous selenium (a-Se) convert X-rays directly or indirectly into electrical signals. They offer higher detective quantum efficiency (DQE) than film-screen systems, meaning they produce high-quality images at lower doses. Digital detectors also enable noise reduction algorithms, such as multi-frequency processing and temporal averaging, which suppress random noise while preserving edges. Proper calibration and detector maintenance, including flat-field correction, remove fixed-pattern noise from defective pixels or gain variations.
11. Balancing Patient Dose and Image Quality |
All techniques for improving image quality must be weighed against the ALARA principle (As Low As Reasonably Achievable) for radiation protection. Techniques such as raising mAs or kVp, using a grid, or selecting a slow intensifying screen can increase dose. A risk-benefit assessment is essential. Justification of each X-ray request, use of protective shielding, tailored exposure parameters, and digital post-processing all contribute to an optimised imaging chain where diagnostic quality is maintained while keeping dose to the minimum necessary.
